老年女性压力性尿失禁患者盆膈裂孔的超声影像学表现分析

摘　　要：目的:研究老年女性压力性尿失禁(SUI)患者盆膈裂孔超声影像学的表现。方法:选取50例老年女性SUI患者为研究组,选择同期50例非SUI老年女性为对照组。采用经会阴实时三维超声检查,比较两组在静息状态、Valsalva动作及缩肛运动下盆膈裂孔声像图表现及变化,分析盆膈裂孔前后径(LH-AP)、盆膈裂孔横径(LH-T)、盆膈裂孔面积(LH-A)以及耻骨内脏肌厚度(T)等指标。结果:静息状态下,研究组LH-T和LH-A大于对照组,具有统计学意义(P<0.05);Valsalva动作下,研究组LH-AP和LH-A大于对照组,T小于对照组,均有统计学意义(P<0.05);缩肛动作时,研究组与对照组比较LH-AP、LH-T和LH-A,均具有统计学意义(P<0.05)。结论:实时三维超声能够对老年女性SUI患者的盆膈裂孔形状改变进行有效评估,对老年女性SUI的诊断提供依据。Objective:This paper is to study the ultrasonographic manifestations of levator hiatus in elderly women with stress urinary incontinence.Methods:In this study,there were 50 elderly female SUI patients as the study group,while 50 cases excluded from SUI were selected as the control group.Real-time three-dimensional ultrasound via perineum was performed to all women,the two groups were compared at rest,Valsalva state and contraction,assess parameters as anterior and posterior diameter of levator hiatus(LH-AP),transverse diameter(LH-T),area(LH-A),as well as the pubic visceral muscle thickness(T).Results:At the resting state,LH-T and LH-A in study group than in the control group,with statistically differences(P<0.05).Under the condition of Valsalva,LH-AP and LH-A in the study group than in the control group,and T was lower than another group(P<0.05).On contraction state,there were statistically differences between study group and the control group at LH-AP,LH-T and LH-A(P<0.05).Conclusions:The real-time three-dimensional ultrasound via perineum is able to effectively evaluate the morphological changes of levator hiatus in elderly women with SUI,which played an important role in the diagnosis of SUI in elderly women.
