基于批判性思维的持续性改进护理联合中医坐浴对痔疮术后患者健康行为、疼痛应激和病耻感的影响

摘　　要：目的 探讨基于批判性思维的持续性改进护理联合中医坐浴对痔疮术后患者健康行为、疼痛应激和病耻感的影响。方法 将我院2021年5月至2022年12月收治的88例痔疮术后患者随机分为对照组(n=44,中医坐浴)和联合组(n=44,在对照组基础上联用批判性思维的持续性改进护理)。比较护理前、护理1 d、护理2 d后两组患者疼痛程度[视觉模拟疼痛评分量表(VAS)]、健康行为[健康促进生活方式量表(HPLP)]、应激指标[收缩压(SBP)、舒张压(DBP)、心率(HR)]和病耻感[Skidmore焦虑病耻感量表(SASS)],并对两组患者护理满意度进行评估。结果 护理后,两组患者VAS评分、病耻感评分、SBP、DBP、HR水平较护理前显著降低(P <0.05),且联合组均低于对照组(P <0.05);两组患者HPLP健康责任、体育运动、营养、精神成长、压力管理、人际关系评分较护理前均显著增加(P <0.05),且联合组均高于对照组(P <0.05);联合组护理满意度显著高于对照组(P <0.05)。结论 基于批判性思维的持续性改进护理联合中医坐浴可有效减轻痔疮术后患者疼痛症状,改善其健康行为、应激症状及病耻感,并可获得满意的护理效果。Objective To explore the effect of continuous improvement nursing based on critical thinking combined with traditional Chinese medicine hip bath on health behavior,pain stress and sense of shame of patients after hemorrhoid surgery.Methods The 88 patients with hemorrhoids admitted to our hospital from May 2022 to December 2022 were randomly divided into a control group(n=44,Chinese medicine hip bath)and a combined group(n=44,on the basis of the control group,continuous improvement nursing with critical thinking).Compare the pain levels of two groups of patients before nursing,1 day of nursing,and 2 days after nursing[Visual Analog Pain Scale(VAS)],compare the health behaviors of the two groups of patients before and after nursing[Health Promotion Lifestyle Scale(HPLP)],stress indicators[systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR)],and shame[Skidmore Anxiety and Shame Scale(SASS)],and evaluate the nursing satisfaction of the two groups of patients.Results After nursing care,the VAS score,sense of shame scores SBP,DBP,and HR levels of the two groups of patients significantly decreased(P<0.05),and the combined group was lower than the control group(P<0.05).HPLP health responsibility,sports,nutrition,spiritual growth,stress management,interpersonal relationship scores of patients in both groups were significantly increased(P<0.05),and the combined group was higher than the control group(P<0.05).The nursing satisfaction of the combined group was significantly higher than that of the control group(P<0.05).Conclusion Continuous improvement nursing based on critical thinking combined with traditional Chinese medicine hip bath can effectively alleviate the pain symptoms of patients after hemorrhoid surgery,improve their health behavior,stress symptoms and sense of shame,and obtain satisfactory nursing effect.
