摘 要:目的分析磁共振成像(MRI)联合肿瘤标志物诊断卵巢癌的受试者工作曲线(ROC)。方法回顾性收集2018年1月~2022年6月南通市肿瘤医院收治的30例卵巢癌患者临床资料作为恶性组,选取同期在我院接受治疗的43例卵巢良性肿瘤患者临床资料作为良性组,所有患者均经手术病理检查确诊,并进行MRI检查,比较两组MRI影像学表现及血清肿瘤标志物水平,统计MRI对良恶性卵巢肿瘤的诊断情况,采用ROC曲线分析MRI与肿瘤标志物单独及联合对卵巢癌的诊断价值。结果恶性组双侧分布、形态不规则、边界不清晰、肿瘤成分为囊实性或实性、强化明显患者占比(76.67%、83.33%、80.00%、100.00%、100.00%)高于良性组(11.63%、18.60%、16.28%、9.30%、11.63%,P<0.05)。MRI对良性卵巢肿瘤的检出率为86.05%,对恶性卵巢肿瘤的检出率为93.33%。恶性组血清糖类抗原199(CA199)、糖类抗原125(CA125)、人附睾蛋白4(HE4)、甲胎蛋白(AFP)水平高于良性组(P<0.05)。MRI与肿瘤标志物联合诊断卵巢癌的曲线下面积(AUC,0.914)高于MRI、血清CA199、CA125、HE4、AFP单独诊断(0.897、0.842、0.737、0.791、0.738),MRI诊断卵巢癌的AUC高于血清CA199、CA125、HE4、AFP单独诊断,MRI与肿瘤标志物联合、MRI诊断卵巢癌的敏感度(90.00%、93.33%)高于血清CA199、CA125、HE4、AFP单独诊断(73.33%、70.00%、80.00%、66.67%,P<0.05)。结论卵巢癌患者MRI检查影像学表现主要为双侧分布、形态不规则、边界不清晰、肿瘤成分为囊实性或实性、强化明显,其联合血清肿瘤标志物可提高对卵巢癌患者的诊断价值。Objective To analyze the receiver operating characteristic(ROC)curve of magnetic resonance imaging(MRI)combined with tumor markers in the diagnosis of ovarian cancer.Methods The clinical data of 30 patients with ovarian cancer admitted to Nantong Cancer Hospital from January 2018 to June 2022 were retrospectively collected as the malignant group,and the clinical data of 43 patients with benign ovarian tumors treated in our hospital during the same period were selected as the benign group.All patients were confirmed by surgical pathological examination and underwent MRI examination.The MRI imaging manifestations and serum tumor markers were compared between the two groups.The diagnosis of benign and malignant ovarian tumors by MRI was statistically analyzed.The diagnostic value of MRI and tumor markers alone and in combination for ovarian cancer was analyzed by ROC curve.Results The proportion of patients with bilateral distribution,irregular shape,unclear boundary,cystic or solid tumor components and obvious enhancement in the malignant group(76.67%,83.33%,80.00%,100.00%,100.00%)was higher than that in the benign group(11.63%,18.60%,16.28%,9.30%,11.63%,P<0.05).The detection rate of benign ovarian tumors was 86.05%,and the detection rate of malignant ovarian tumors was 93.33%.The serum levels of carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),human epididymal protein 4(HE4)and alpha-fetoprotein(AFP)in the malignant group were higher than those in the benign group(P<0.05).The area under the curve(AU C,0.914)of MRI combined with tumor markers in the diagnosis of ovarian cancer was higher than that of MRI,serum CA199,CA125,HE4 and AFP alone(0.897,0.842,0.737,0.791,0.738).The AU C of MRI in the diagnosis of ovarian cancer was higher than that of serum CA199,CA125,HE4 and AFP alone.The sensitivity of MRI combined with tumor markers and MRI in the diagnosis of ovarian cancer(90.00%,93.33%)was higher than that of serum CA199,CA125,HE4,AFP alone(73.33%,70.00%,80.00%,66.67%,P<0.05).Conclusion MRI imaging fi
