摘  要:目的探讨垂直于肛管长轴的斜轴位在肛瘘诊断中的应用价值。方法选取上海健康医学院附属崇明医院2019年1月-2022年6月期间,临床拟诊为肛瘘并具有MRI普通轴位和斜轴位的两种扫描结果的123名患者,并与术后病理金标准比较,构建受试者工作特征(ROC)曲线,探讨二种扫描方法对肛瘘开口数、瘘管数、瘘管分支数及脓肿数的诊断价值。结果斜轴位、普轴位对肛瘘开口数(单位:个)、瘘管数、瘘管分支数及脓肿数总的准确诊断个数、漏诊个数及误诊个数分别为262、11、12和246、27、23,二组的总灵敏度、特异度、准确度及曲线下面积分别为96.0、95.6、95.79、0.951和90.1、91.6、90.84及0.905,二组差异有统计学意义(P<0.01)。结论MR斜轴位扫描有利于肛瘘内口、瘘管、瘘管及脓肿的准确显示,有利于提高影像诊断的准确性。Objective To explore the application value of the oblique axis bits perpendicular to the anal pipe's long shaft in the diagnosis of anal fistula.Methods From January 2019 to June 2022,Chongming Hospital affiliated to Shanghai College was selected,and 123 patients were clinically diagnosed with two scanning results with anal fistula and had MRI ordinary axis and oblique axis.Standard comparison,build a subject(ROC)curve of the subject(ROC),and explore the diagnostic value of the two scanning methods to the number of anal fistula,the number of fistula,the number of fistulas,and the number of abscesses.Results The number of oblique axis,the number of anal fistula,the number of fistula,the number of fistula branches and the total number of abscesses,the number of missed diagnosis,and the number of misdiagnosis were 262,11,12,and the number of misdiagnosed diagnosis.246,27,23,the total sensitivity,specificity,accuracy,and curves of the second group are 96.0,95.6,95.79,0.951 and 90.6,90.84 and 0.905.To.Conclusion The MR oblique axis scan is conducive to the accurate display of the anal fistula,fistula,fistula and abscess,and it is conducive to improving the accuracy of image diagnosis.
