自制冰硝散外敷联合冰敷治疗闭合性骨折肿胀疼痛40例临床观察

摘　　要：目的:分析应用自制冰硝散外敷联合冰敷治疗闭合性骨折肿胀疼痛的疗效。方法:选取80例闭合性骨折肿胀疼痛患者,采用随机、双盲的分配方法分为对照组与观察组各40例。对照组采取常规消肿止痛治疗,具体方法为常规冰敷、静脉用药七叶皂苷钠与甘露醇;观察组采用自制冰硝散外敷联合冰敷治疗。观察记录两组治疗前及治疗后7天患肢疼痛评分(VAS评分)、肿胀消除率治疗前及治疗后3天、7天肿胀值,出现皮纹所用治疗天数,肿胀并发症发生情况以及镇痛药物使用量。结果:对照组与观察组在治疗后第3天患肢周径变化改变的平均值差值,具有统计学差异(P<0.05),说明冰硝散联合冰敷的消肿效果开始显现;而在第7天患肢肿胀平均值差值,具有明显统计学差异(P<0.05),说明冰硝散外敷联合冰敷的消肿有效果;而在第7天观察组的疼痛平均值的差值亦显著,具有统计学意义(P<0.05)。说明患者在肿胀明显消除的情况下,其炎性反应均有不同程度减轻。结论:自制冰硝散外敷联合冰敷可缓解闭合性骨折患者肿胀和减轻疼痛;该法取材方便,操作简单,适宜在基层医院推广应用。Objective To analyze the efficacy of applying homemade ice-nitrate powder external application combined with ice application for the treatment of swelling and pain of closed fracture.Methods According to the principle of randomization,check the random number table is divided into control group and observation group,two groups for observation.There were 40 cases in the control group and 40 cases in the observation group,with a total of 80 effective observation cases in both groups.The control group was divided into the control group and the observation group by randomized,double-blind allocation method,and the control group was treated with conventional swelling and pain relief treatment,which consisted of conventional ice packs,intravenous medication of sodium heptadecyl saponin and mannitol.The observation group was treated with homemade ice-nitrosan external application combined with ice packs.Observation records of the two groups of patients before treatment and 7 days after the treatment of the affected limb pain score(VAS score),swelling elimination rate before treatment and 3 days after the treatment,7 days after the treatment of swelling value,the number of days used in the treatment of the appearance of skin tattoos,the occurrence of swelling complications and the use of analgesic drugs.Results After t-test,it was found that the difference of the mean value of the change in the circumference of the affected limbs between the control group and the observation group on the 3rd day after treatment was P<0.05,which was statistically different,indicating that the swelling elimination effect of the ice-nitrosan combined with the ice application began to appear;and the difference of the mean value of the swelling of the affected limbs on the 7th day was P<0.05,which was statistically significant,indicating that the swelling elimination of the ice-nitrosan external application combined with the ice application had an effect;and the difference of the average value of the pain in the observation group on the 7th da
