摘 要:目的应用自动纤维定量(AFQ)分析系统性红斑狼疮(SLE)患者白质纤维束改变与其认知功能障碍的关系。方法对39例女性SLE患者(SLE组)和44名年龄、性别及受教育年限相匹配的健康志愿者(HC组)采集头部弥散张量成像(DTI),并行简易精神状态量表(MMSE)评分。应用AFQ分析全脑20条白质纤维束并进行追踪。将追踪成功的18条白质纤维束以等距离分为100个节段,获得每个节段各向异性分数(FA)及平均弥散系数(MD);比较组间各白质纤维束节段FA及MD的差异,分析SLE组与HC组存在差异白质纤维束节段的DTI参数与MMSE评分的相关性。结果相比HC组,SLE组左侧下额枕束(IFOF,96~100节段)及左侧下纵束(ILF,69~79节段)FA降低,而左侧丘脑放射束(1~11节段)、左侧皮质脊髓束(21~24、87~100节段)、右侧皮质脊髓束(20~25、70~90、94~100节段)、右侧扣带束扣带回(45~54节段)、胼胝体小钳(14~19、23~31、68~91节段)、左侧IFOF(8~12、16~21、27~31、92~97节段)、右侧IFOF(10~19节段)、右侧ILF(28~34、43~46节段)及左侧弓形束(38~42节段)MD升高(FDR校正P均<0.05)。SLE患者左侧IFOF 97~100节段FA与MMSE评分均呈正相关(rs=0.336~0.356,P均<0.05)。结论SLE患者左侧IFOF等脑白质纤维束局部节段受损,且与其认知功能改变相关。Objective To analyze changes of white matter fiber bundle in systemic lupus erythematosus(SLE)patients and correlations with cognitive dysfunction using automatic fiber quantification(AFQ).Methods Head diffuse tensor imaging(DTI)of 39 female SLE patients(SLE group)and 44 healthy volunteers with matched age,gender and years of education(HC group)were prospectively enrolled and scored with mini-mental state scale(MMSE).Totally 20 brain white matter fiber bundles were analyzed and tracked based on AFQ.Eighteen white matter fiber bundles were successfully tracked and divided into 100 segments at equal distances,and fractional anisotropy(FA)and the mean diffusivity(MD)of each segment were obtained.FA and MD of each white matter fiber bundle segment were compared between groups,and the relationships of DTI parameters of white matter fiber bundle segments being different between groups of SLE patients with MMSE scores were analyzed.Results In SLE group,FA of left inferior fronto-occipital fasciculus(IFOF,96—100 segments)and left inferior longitudinal fasciculus(ILF,69—79 segments)decreased,while MD of left thalamic radiation(1—11 segments),left corticospinal tract(21—24,87—100 segments),right corticospinal tract(20—25,70—90,94—100 segments),right cingulum cingulate(45—54 segments),forceps minor(14—19,23—31,68—91 segments),left IFOF(8—12,16—21,27—31,92—97 segments),right IFOF(10—19 segments),right ILF(28—34,43—46 segments)and left arcuate fasciculus(38—42 segments)increased compared with those of HC group(all P<0.05,FDR correction).FA of left IFOF 97—100 segments in SLE patients were positively correlated with MMSE scores(r s=0.336—0.356,all P<0.05).Conclusion Local segments of white matter fiber bundles such as left IFOF in SLE patients were damaged,which associated with cognitive function changes.
