基于数据挖掘分析国医大师李佃贵治疗结肠癌术后用药规律

摘　　要：目的:基于古今医案云平台探索国医大师李佃贵治疗结肠癌术后用药规律。方法:于河北省中医院门诊收集2020年1月1日至2022年7月31日经李教授治疗的结肠癌术后患者处方。通过Excel 2021建立数据库并导入古今医案云平台(V2.3.5),经过标准化后进行药物频次、药物属性、药物聚类、药物关联及复杂网络分析,探索李佃贵国医大师治疗结肠癌术后用药规律。结果:共纳入处方121首,通过数据挖掘得出高频用药为黄连、黄芩、半枝莲、白花蛇舌草等,用药以寒、温性为主,味多苦、甘、辛,归经以脾、肝、胃为主。聚类分析得到4组药物,复杂网络分析得到核心处方:黄芩、黄连、白花蛇舌草、半枝莲、当归、白芍、苦参、槟榔、川芎、白术。结论:李教授主张以化浊解毒、宣瘀通络治疗结肠癌术后患者。Objective:to explore the medication patterns of Chinese medicine master Li Diangui in the treatment of colon cancer after surgery based on the ancient and modern medical case cloud platform.Method:We collected prescriptions from postoperative colon cancer patients treated by Professor Li from January 1,2020 to July 31,2022 at the outpatient department of Hebei Provincial Hospital of Traditional Chinese Medicine.The database was established through Excel 2021 and imported into the ancient and modern medical record cloud platform(V2.3.5).After standardization,the drug frequency,drug attribute,drug clustering,drug association and complex network analysis were carried out to explore the medication patterns of Li Diangui,a Chinese medical master,after colon cancer surgery.Results:Totally 121 prescriptions were included.Data mining showed that the high frequently used medicinals were huang lian(Coptidis Rhizoma),huang qin(Scutellariae Radix),ban zhi lian(Scutellariae Barbatae Herba),bai hua she she cao(Agkistrodon),etc.,which were mainly cold or warm in nature,bitter,sweet,or pungent in flavor,belonging to spleen,liver,or stomach.Cluster analysis obtained four groups of medicinals,and complex network analysis obtained the core prescription:huang qin(Scutellariae Radix),huang lian(Coptidis Rhizoma),bai hua she she cao(Agkistrodon),ban zhi lian(Scutellariae Barbatae Herba),dang gui(Angelicae Sinensis Radix),bai shao(Paeoniae Alba Radix),ku shen(Sophorae Flavescentis Radix),bing lang(Arecae Semen),chuan xiong(Chuanxiong Rhizoma) and bai zhu(Atractylodis Macrocephalae Rhizoma).Conclusion:Professor Li advocates the treatment of postoperative patients with colon cancer by eliminating turbid and expelling toxin,diffusing stasis and unblocking collaterals.
