摘 要:目的探讨青中年健康体检人群腰椎骨密度(bone mineral density,BMD)与血糖、血脂、尿酸(uricacid,UC)之间的关系。方法选取健康体检资料完整的青中年282例,根据年龄段分3组,21~30岁组(17例),31~40岁组(112例),41~50岁组(153例),探讨不同年龄组腰椎BMD与身高、体质量、体质量指数(body mass index,BMI)、血脂、血糖、UC的相关性,采用多元logistic回归分析法分析青中年低骨量的影响因素。结果21~30岁组腰椎BMD与血脂、血糖、UC无相关性,31~40岁组腰椎BMD与UC呈负相关(r=-0.192,P=0.042),41~50岁组腰椎BMD与体质量(r=0.180,P=0.026)、BMI(r=0.167,P=0.039)呈正相关。多元logistic回归分析显示,BMI(OR=0.846,95%CI:0.762~0.940)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)(OR=0.241,95%CI:0.060~0.962)是青中年低骨量的保护性因素,UC(OR=1.006,95%CI:1.002~1.009)是青中年低骨量的独立危险因素,血糖、甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)、HDL-C、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)与青中年低骨量的发生无相关(P>0.05)。结论UC升高是青中年低骨量的独立危险因素,对于青中年人群应重视高UC的管理,预防骨质疏松症的发生。Objective To investigate the relationship between lumbar bone mineral density(BMD)and blood glucose,lipids,and uric acid(UC)in young and middle-aged health check-up population.Methods ResultsConclusion Methods A total of 282 cases of young and middle-aged with complete health physical examination data were selected and divided into 3 groups according to age,21-30 years group(17 cases),31-40 years group(112 cases),and 41-50 years group(153 cases).Then we explored the correlation between lumbar spine BMD and height,body mass,body mass index(BMI),blood lipids,blood glucose and UC in different age groups,and analyzed the influencing factors of low bone mass in young and middle-aged using multiple logistic regression analysis.Results There was no correlation between lumbar BMD and blood lipids,blood glucose and UC in the 21-30 years group.However,there was a negative correlation between lumbar BMD and UC in the 31-40 years group(r=-0.192,P=0.042),and a positive correlation between lumbar BMD and body mass(r=0.180,P=0.026)and BMI(r=0.167,P=0.039)in the 41-50 years group.Multiple logistic regression analysis showed that BMI(OR=0.846,95%CI:0.762-0.940),high density lipoprotein cholesterol(HDL-C)(OR=0.241,95%CI:0.060-0.962)were the protective factors for low bone mass in young and middleaged adults,and uric acid(OR=1.006,95%CI:1.002-1.009)was an independent risk factor for low bone mass in young and middle-aged adults.However,blood glucose,triglycerides(TG),total cholesterol(TC),HDL-C,and low density lipoprotein cholesterol(LDL-C)were not associated with the occurrence of low bone mass in young and middle-aged adults(P>0.05).Conclusion Elevated uric acid is an independent risk factor for low bone mass in young and middle-aged people,and attention should be paid to the management of high UC and prevention of osteoporosis in the young and middle-aged population.
