体外冲击波联合艾灸治疗冻结期肩周炎的临床效果

摘　　要：目的观察体外冲击波联合艾灸治疗冻结期肩周炎的临床效果。方法回顾性分析2019年1月至2021年1月本院收治的126例冻结期肩周炎患者的临床资料,按治疗方案的不同将其分为对照组(68例,推拿按摩联合超短波治疗)和试验组(58例,体外冲击波联合艾灸治疗)。比较两组的治疗效果。结果治疗前,两组的视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);治疗后,试验组的VAS评分低于对照组(P<0.05)。治疗前,两组的前列腺素E_(2)(PGE_(2))、P物质(SP)和5-羟色胺(5-HT)水平比较,差异无统计学意义(P>0.05);治疗后,试验组的PGE_(2)、SP和5-HT水平均低于对照组(P<0.05)。治疗前,两组的日常生活能力(ADL)和关节活动度(ROM)评分比较,差异无统计学意义(P>0.05);治疗后,试验组的ADL、ROM评分均高于对照组(P<0.05)。试验组的治疗总有效率高于对照组(P<0.05)。试验组的复发率低于对照组(P<0.05)。结论体外冲击波联合艾灸治疗冻结期肩周炎的效果显著,能够减轻患者疼痛,降低血清疼痛介质水平,改善肩关节功能,且复发率低,值得临床推广。Objective To observe the clinical effect of extracorporeal shock wave combined with moxibustion in the treatment of scapulohumeral periarthritis during frozen period.Methods The clinical data of 126 patients with scapulohumeral periarthritis during frozen period admitted in our hospital from January 2019 to January 2021 were retrospectively analyzed,and the patients were divided into control group(68 cases,massage combined with ultra-short wave treatment)and experimental group(58 cases,extracorporeal shock wave combined with moxibustion treatment)according to different treatment plan.The therapeutic effects of the two groups were compared.Results Before treatment,there was no significant difference in the Visual Analogue Scale(VAS)score between the two groups(P>0.05);after treatment,the VAS score of the experimental group was lower than that of the control group(P<0.05).Before treatment,there were no significant differences in the levels of prostaglandin E_(2)(PGE_(2)),substance P(SP)and 5-hydroxytryptamine(5-HT)between the two groups(P>0.05);after treatment,the levels of PGE_(2),SP and 5-HT in the experimental group were lower than those in the control group(P<0.05).Before treatment,there were no significant differences in the Activities of Daily Living(ADL)and Range of Motion(ROM)scores between the two groups(P>0.05);after treatment,the ADL and ROM scores of the experimental group were higher than those of the control group(P<0.05).The total effective rate of treatment in the experimental group was higher than that in the control group(P<0.05).The recurrence rate of the experimental group was lower than that of the control group(P<0.05).Conclusion Extracorporeal shock wave combined with moxibustion in the treatment of scapulohumeral periarthritis during frozen period has significant effect,it can relieve the pain of patients,reduce the level of pain mediators in serum,improve the shoulder joint function,with low recurrence rate,which is worthy of clinical promotion.
