纳布啡与右美托咪定对腰硬联合麻醉下剖宫产术中牵拉痛与寒战的防治效果比较

摘　　要：目的 比较纳布啡与右美托咪定对腰硬联合麻醉下剖宫产术中牵拉痛与寒战的防治效果。方法 选取2021年3月至8月在我院行剖宫产术的400例产妇为研究对象,将其随机分对照组和观察组,各200例。两组均接受腰硬联合麻醉下剖宫产术,在胎儿娩出断脐后,对照组静脉注射右美托咪定,观察组静脉注射纳布啡。比较两组的临床效果。结果 给药10 min(T_(2))、术毕(T_(3)),两组的平均动脉压(MAP)、心率(HR)低于给药前(T_(0))(P<0.05)。观察组的术中牵拉痛总发生率低于对照组(P<0.05)。观察组的术中寒战总发生率低于对照组(P<0.05)。术后12、24、48 h,观察组的视觉模拟评分法(VAS)评分低于对照组(P<0.05)。结论 与右美托咪定比较,纳布啡用于腰硬联合麻醉下剖宫产术可取得更为理想的防治效果,值得临床推广与应用。Objective To compare the prevention and treatment effects of nalbuphine and dexmedetomidine on traction pain and chills during cesarean section under combined spinal-epidural anesthesia.Methods From March to August 2021,400 parturients who underwent cesarean section in our hospital were selected as the research objects,and the parturients were randomly divided into control group and observation group,with 200 cases in each group.Both groups received cesarean section under combined spinal-epidural anesthesia,after the fetus was delivered and the umbilical cord was severed,the control group was intravenously injected with dexmedetomidine,and the observation group was intravenously injected with nalbuphine.The clinical effects of the two groups were compared.Results At 10 min after administration(T_(2))and at the end of operation(T_(3)),the mean arterial pressure(MAP)and heart rate(HR)of the two groups were lower than those before administration(T_(0))(P<0.05).The total incidence of intraoperative traction pain in the observation group was lower than that in the control group(P<0.05).The total incidence of intraoperative chills in the observation group was lower than that in the control group(P<0.05).At 12,24 and 48 h after operation,the Visual Analogue Scales(VAS)score of the observation group was lower than that of the control group(P<0.05).Conclusion Compared with dexmedetomidine,nalbuphine can achieve more ideal prevention and treatment effect for cesarean section under combined spinal-epidural anesthesia,which is worthy of clinical promotion and application.
