血府逐瘀汤预防上肢皮瓣术后血管危象的临床研究

摘　　要：目的:观察血府逐瘀汤预防上肢皮瓣术后血管危象的临床疗效。方法:选取2022年1月至2023年1月就诊于赣州市南康区第一人民医院的60例行上肢皮瓣转移术的患者,采用简单数字随机法分为观察组和对照组,各30例。术后对照组患者常规应用“三抗”治疗,观察组患者在“三抗”的基础上加服血府逐瘀汤剂,比较两组患者术后皮瓣的血运情况、总体疗效。结果:术后3 d、7 d,观察组患者皮瓣肿胀程度优于对照组,差异具有统计学意义(Z分别为-2.28、-3.46,P<0.05);术后1 d、3 d、7 d,观察组患者毛细血管反应优于对照组,差异具有统计学意义(Z分别为-6.89、-6.29、-7.35,P<0.05);观察组患者疗效优于对照组,差异具有统计学意义(Z=-3.44,P<0.05)。结论:上肢皮瓣术后的患者在常规“三抗”的基础上,加用血府逐瘀汤可有效减少血管危象的发生,提高皮瓣成活率。Objective To observe the clinical efficacy of Xuefu Zhuyu decoction in preventing vascular crisis after upper limb flap surgery.Methods Sixty patients undergoing upper limb flap transfer in Ganzhou City Nankang District First People's Hospital from January 2022 to January 2023 were selected and divided into an observation group and a control group by simple numerical random method,with 30 cases in each group.After operation,the control group was treated with"three antibodies",and the observation group was treated with Xuefu Zhuyu decoction on the basis of"three antibodies".The blood flow and overall efficacy of the postoperative flap were compared between the two groups.Results 3 days and 7 days after surgery,the flap swelling degree of patients in the observation group was better than that in the control group,and the difference was statistically significant(Z=-2.28,-3.46,P<0.05).On day 1,day 3 and day 7 after surgery,the capillary response in the observation group was better than that in the control group,and the difference was statistically significant(Z=-6.89,-6.29,-7.35,P<0.05).The efficacy of the observation group was better than that of the control group,and the difference was statistically significant(Z=-3.44,P<0.05).Conclusion On the basis of conventional"three antibodies",Xuefu Zhuyu decoction can effectively reduce the occurrence of vascular crisis and increase the survival rate of skin flap.
