摘　　要：目的:探讨解剖钢板结合不同旋前方肌入路治疗桡骨远端骨折(DRF)的临床效果。方法:将49例DRF患者随机分为两组,对照组24例采用Henry入路旋前方肌下骨膜外置入钢板,观察组25例采用劈开肱桡肌旋前方肌下置入钢板。比较两组手术指标、腕关节功能及活动度。结果:观察组钢板调放次数少于对照组,骨折愈合时间短于对照组,GW评分低于对照组,关节活动度大于对照组,差异均有统计学意义(P<0.05)。两组切口长度及手术时间比较,差异均无统计学意义(P>0.05)。结论:DRF患者采用劈开肱桡肌旋前方肌下置入钢板可减少术中钢板调放次数,缩短骨折愈合时间,提高治疗效果。Objective:To explore the clinical effect of anatomical plate combined with different pronator muscle approaches in the treatment of distal radius fracture(DRF).Methods:49 patients with DRF were randomly divided into two groups.24 patients in the control group were treated with external placement of the anterior subperiosteal circumflex plate using the Henry approach,while 25 patients in the observation group were treated with external placement of the split brachioradialis circumflex plate.The operative index,wrist function and mobility were compared between the two groups.Results:In the observation group,the number of plate adjustments was less than that in the control group,the fracture healing time was shorter than that in the control group,the GW score was lower than that in the control group,and the joint mobility was greater than that in the control group,with statistically significant difference(P<0.05).Incision length and operation time were not significantly different between the two groups(P>0.05).Conclusion:The placement of steel plate under the pronator muscle of the split brachioradialis muscle in patients with DRF can reduce the number of plate adjustments during operation,shorten the fracture healing time,and improve the treatment effect.
