摘　　要：目的探讨Vitapex糊剂联合盐酸米诺环素根管治疗根尖周炎伴牙周炎的临床疗效。方法前瞻性选取医院2020年1月至2021年12月收治的根尖周炎伴牙周炎患者112例(患牙112颗),按随机抽签法分为观察组和对照组,各56例。两组均采用根管治疗,对照组常规采用棉签及碘酚为填充材料,牙周袋注碘甘油;观察组以Vitapex糊剂为填充材料,牙周袋给予盐酸米诺环素软膏;均治疗4周。结果观察组总有效率为94.64%,显著高于对照组的80.36%(P<0.05);观察组患者治疗后的牙周袋深度、龈沟出血指数、牙龈指数、菌斑指数均显著低于对照组,龈沟液中的C反应蛋白、肿瘤坏死因子-α、白细胞介素(IL)-1β、IL-6水平均显著低于对照组,视觉模拟量表评分及口腔健康影响程度量表评分均显著低于对照组(P<0.05);观察组与对照组的不良反应发生率相当(8.93%比5.36%,P>0.05)。结论Vitapex糊剂联合盐酸米诺环素根管治疗根尖周炎伴牙周炎,能有效改善患者的牙周组织状况,降低龈沟液的炎性因子水平,缓解疼痛,提高患者的生存质量。Objective To investigate the clinical efficacy of Vitapex paste combined with minocycline hydrochloride root canal therapy for periapical periodontitis with periodontitis.Methods A total of 112 patients with periapical periodontitis and periodontitis(involving 112 teeth)admitted to the hospital from January 2020 to December 2021 were prospectively selected and divided into the observation group and the control group by the random lottery method,with 56 cases in each group.The patients in the two groups were treated with root canal therapy.The patients in the control group were routinely treated with cotton swab and iodophenol as the filling materials,and iodine glycerol was injected into the periodontal pocket.The patients in the observation group were treated with Vitapex paste as the filling materials,and Minocycline Hydrochloride Ointment was injected into the periodontal pocket.Both groups were treated for four weeks.Results The total effective rate in the observation group was 94.64%,which was significantly higher than 80.36%in the control group(P<0.05).After treatment,the periodontal pocket depth(PD),gingival sulcus bleeding index(BI),gingival index(GI)and plaque index(PLI)in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin(IL)-1βand IL-6 levels in gingival sulcus fluid in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the scores of Visual Analog Scale(VAS)and Oral Health Impact Profile-14(OHIP-14)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(8.93%vs.5.36%,P>0.05).Conclusion Vitapex paste combined with minocycline hydrochloride root canal therapy for periapical periodontitis with periodontitis can effectively improve the periodontal tissue status,decrease the inflammatory factor level
