摘　　要：目的探讨右美托咪定对腹腔镜下全子宫+双附件切除术全身麻醉患者苏醒质量及麻醉深度的影响。方法选取59例接受腹腔镜下全子宫+双附件切除术治疗的患者,按照随机数字表法分为对照组(29例)与研究组(30例)。对照组麻醉诱导常规药物,研究组采用右美托咪定麻醉。比较两组麻醉效果。结果研究组在入室时刻(T_(1))、离开麻醉恢复室(PACU)时刻(T_(4))的(HR)、平均动脉压(MAP)及脑电双频指数(BIS)水平与对照组比较,差异无统计学意义(P>0.05),而研究组在切皮时刻(T_(2))、拔除喉罩时刻(T_(3))的HR、MAP、BIS水平均更低(P<0.05)。两组意识恢复时间比较,差异无统计学意义(P>0.05),研究组拔管时间与PACU停留时间短于对照组(P<0.05)。研究组苏醒期躁动不良反应发生率为10.00%,低于对照组的31.03%(P<0.05)。结论在腹腔镜下全子宫+双附件切除术患者全身麻醉中应用右美托咪定,可以有效维持患者术中的生命体征稳定,提高患者的麻醉苏醒质量,降低患者苏醒期不良反应,值得临床推广。Objective To investigate the effects of dexmedetomidine on the quality of recovery and depth of anesthesia in patients with general anesthesia after laparoscopic total hysterectomy and double adnexal resection.Methods 59 patients who underwent laparoscopic total hysterectomy plus double adannexectomy were selected and divided into control group(29 cases)and study group(30 cases)according to random number table method.The control group was anesthetized with conventional drugs,while the study group was anesthetized with dexmedetomidine.The anesthetic effect of the two groups was compared.Results There were no significant differences in the levels of HR,MAP and BIS between the study group and the control group at the time of entry(T_(1))and the time of leaving the anesthesia recovery room(PACU)(T_(4))(P>0.05).The levels of HR,MAP and BIS in the study group were lower at the incision time(T_(2))and the laryngeal mask removal time(T_(3))(P<0.05).There was no significant difference in consciousness recovery time between the two groups(P>0.05).Extubation time and PACU residence time in the study group were shorter than those in the control group(P<0.05).The incidence of restlessness adverse reaction in the study group was 10.00%,which was lower than 31.03% in the control group(P<0.05).Conclusion The application of dexmedetomidine in general anesthesia in patients with laparoscopic total hysterectomy plus double adnexal resection can effectively maintain the stability of patients'vital signs during the operation,improve the quality of anesthesia recovery and reduce the adverse reactions during recovery,which is worthy of clinical promotion.
