关节镜辅助闭合复位经皮内固定治疗内踝骨折的疗效观察

摘　　要：目的观察踝关节镜辅助下闭合复位并经皮治疗内踝骨折的效果。方法2019年1月至2021年6月单纯内踝骨折患者20例,均采用关节镜下闭合复位经皮内固定。受伤至手术时间为3~8d。随访观察术后并发症情况、骨折愈合情况、术后1年疼痛视觉模拟(VAS)评分、美国足踝外科协会(AOFAS)评分和AOFAS优良率情况。结果19例患者获得随访,1例失访,随访时间12~16个月。术后19例内踝骨折均愈合,愈合时间10~12周。末次随访时患者AOFAS评分为(90.8±4.3)分,AOFAS优13例,良4例,可2例,优良率89.5%。均无骨折延迟愈合、踝关节慢性疼痛及功能性障碍等并发症发生。结论采用踝关节镜下辅助闭合复位经皮内固定治疗内踝骨折愈合率高,并发症少,踝关节功能恢复好,值得临床推广应用。Objective To observe the clinical effect of ankle arthroscopy-assisted closed reduction and percutaneous treatment of medial malleolus fracture.Methods From January 2019 to June 2021,20 patients with simple medial malleolus fracture were treated with arthroscopic closed reduction and percutaneous internal fixation.The time from injury to operation was 3~8 days.Postoperative complications,fracture healing,visual analogue scale of pain(VAS),American foot and ankle Surgical Association score(AOFAS)and AOFAS excellent and good rate were observed in postoperative follow-up.Results In this study,19 patients were followed up and 1 lost follow-up.The follow-up period was 12 to 16 months.All the 19 cases of medial malleolus fracture healed after the operation,and the healing time was 10 to 12 weeks.At the last follow-up,the average score of American and foot-ankle Association was 90.8±4.3.The excellent and good rate of AOFAS was excellent in 13 cases,good in 4 cases,fair in 2 cases,and the excellent and good rate was 89.5%.There were no complications such as delayed union of fracture,chronic pain of ankle joint and functional disorder.Conclusion The use of ankle arthroscopy-assisted closed reduction and percutaneous internal fixation in the treatment of medial malleolus fracture has high healing rate,few complications and good functional recovery of ankle joint,which is worthy of clinical application.
