DSA引导下双导丝辅助与单导丝辅助置入肠梗阻导管治疗急性肠梗阻的疗效比较

摘　　要：目的比较数字减影血管造影(DSA)引导下双导丝辅助与单导丝辅助置入肠梗阻导管治疗急性肠梗阻的疗效。方法回顾性分析2020年4月至2022年3月100例急性肠梗阻患者的临床资料,根据肠梗阻导管置入方法不同分为观察组(n=50)与对照组(n=50),两组患者均在DSA引导下置入经鼻型肠梗阻导管进行保守治疗,其中观察组采用双导丝辅助置管,对照组采用单导丝辅助置管,比较两组各项指标并评价疗效。结果观察组术中一次置管成功率、置管耐受率均高于对照组,置管深度长于对照组,操作时间、X线透视时间短于对照组、造影剂用量均小于对照组(P<0.05);观察组整体治疗有效率高于对照组,并发症发生率低于对照组(P<0.05)。结论DSA引导双导丝辅助置入肠梗阻导管治疗急性肠梗阻,有利于提高置管成功率及患者耐受度,有利于提高肠梗阻保守治疗成功率,值得推广。Objective To compare the efficacy of digital subtraction angiography(DSA)-guided double-guidewire-assisted and single-guidewire-assisted ileus catheterization in treating acute intestinal obstruction.Methods The clinical data of 100 patients with acute intestinal obstruction from April 2020 to March 2022 were retrospectively analyzed.The patients in the two groups were all implanted with nasal ileus catheters under the guidance of DSA for conservative treatment.The observation group was assisted by a double guide wire,and the control group was assisted by the single guide wire.The indexes of the two groups were compared,and the curative effects were evaluated.Results In the observation group,the successful rate of one-time catheterization and the tolerance rate of catheterization were higher than those of the control group,the depth of catheterization was longer than that of the control group,the operation time and X-ray fluoroscopy time were shorter than those of the control group,and the amount of contrast medium was less than that of the control group(P<0.05).The overall effective rate of treatment in the group was higher than that in the control group,and the incidence of complications was lower than that in the control group,with statistical significance(P<0.05).Conclusion DSA-guided double guidewire assisted insertion of intestinal obstruction catheter in the treatment of acute intestinal obstruction is beneficial to improve the success rate of catheterization and patient tolerance.It is conducive to improving the success rate of conservative treatment of intestinal obstruction,which is worthy of promotion.
