经直肠超声造影与磁共振对不同Gleason评分前列腺癌诊断价值比较

摘　　要：目的比较经直肠超声造影与磁共振检查对不同Gleason评分前列腺癌患者的诊断价值。方法回顾性分析2021年1~10月309例临床疑似前列腺癌患者的临床资料,所有患者均行CEUS和MRI检查。根据穿刺病理Gleason评分将前列腺癌患者分为低危、中危、高危三组,比较CEUS与MR对各组的诊断效能。结果CEUS与MR对前列腺癌诊断敏感度分别为75.3%、82.9%,特异度分别为69.9%、70.5%,准确度分别为72.5%、76.4%,差异无统计学意义(P>0.05)。低、中危组中,MR+CEUS检查对前列腺癌的阳性检出率高于CEUS、MR单独检查,差异有统计学意义(P<0.05),MR、CEUS单独使用时对前列腺癌的阳性检出率差异无统计学意义(P>0.05)。高危组中,CEUS+MR检查、MR检查、CEUS检查对前列腺癌阳性检出率比较差异无统计学意义(P>0.05)。结论CEUS和MR对前列腺癌均具有较高的诊断价值,二者联合使用能提高中、低危患者的诊断效能。Objective To compare the diagnostic value of contrast-enhanced ultrasound(CEUS)and magnetic resonance imaging(MRI)in prostate cancer patients with different Gleason scores.Methods Three hundred and nine patients with suspected prostate cancer in our hospital from January 2021 to October 2021 were included.CEUS and MRI examinations were performed on all patients.According to Gleason score,prostate cancer patients were divided into the low-risk,the medium-risk and the high-risk groups,and the diagnostic efficacy of CEUS and MR for each group was compared.Results The sensitivity,specificity and accuracy of CEUS and MR tests for prostate cancer were 75.3%and 82.9%,69.9%and 70.5%,respectively,and 72.5%and 76.4%,respectively.There was no statistical significance between the two groups(P>0.05).In low-risk and medium-risk groups,the positive detection rate of prostate cancer by MR combined with CEUS was higher than that by CEUS and MR alone,with statistical significance(P<0.05).There was no significant difference in the positive detection rate of prostate cancer between the two groups when MR and CEUS were used alone(P>0.05).In high-risk groups,there was no significant difference in the positive detection rate of CEUS combined with MR,MR and CEUS for prostate cancer(P>0.05).Conclusion CEUS and MR have high diagnostic value for prostate cancer,and the combined use of CEUS and MR improves the diagnostic efficacy of low-and medium-risk patients.
