摘 要:目的 探究甲状腺微小乳头状癌组织中CD56、CK19、TPO、HBME-1的表达及与淋巴结转移的关系。方法 选取2020年3月至2022年3月三明市第二医院手术切除经病理确诊的40例伴有淋巴结转移的甲状腺微小乳头状癌组织为A组,40例无淋巴结转移的甲状腺微小乳头状癌组织为B组,40例甲状腺良性病变组织为C组。经免疫组化方法检测及比较3组CD56、CK19、TPO及HBME-1表达情况,采用Spearman相关性分析甲状腺微小乳头状癌组织CD56、CK19、TPO及HBME-1的表达与淋巴结转移的关系。结果 A组及B组CK19及HBME-1阳性表达率显著高于C组,CD56及TPO阳性表达率显著低于C组,差异有统计学意义(P <0.05);A组与B组的CD56、CK19、TPO及HBME-1阳性表达率比较,差异无统计学意义(P> 0.05);Spearman相关性分析显示,甲状腺微小乳头状癌组织CD56、CK19、TPO及HBME-1的表达与淋巴结转移无相关性(P> 0.05)。结论 甲状腺微小乳头状癌组织CD56、CK19、TPO及HBME-1的表达显著异常,且均与淋巴结转移无相关性,在甲状腺微小乳头状癌中有一定的检测价值。Objective To investigate the expression of tissue CD56,CK19,TPO,HBME-1 and their relationship with lymph node metastasi of patients with thyroid micropapillary carcinoma.Methods The tissue of 40 surgical patients with thyroid micropapillary carcinoma complicated with lymph node metastasis confirmed by pathology in Sanming Second Hospital from March 2020 to March 2022 were chosen as the group A,the tissue of 40 cases with thyroid micropapillary carcinoma and without lymph node metastasis at the same time were chosen as the group B,the tissue of 40 cases with benign thyroid disease were chosen as the group C.Then the expression situation of tissue CD56,CK19,TPO and HBME-1 of three groups were detected with immunohistochemical method,then the correlation between tissue CD56,CK19,TPO,HBME-1 and lymph node metastasis of patients with thyroid micropapillary carcinoma were analyzed with spearman correlation analysis.Results The positive expression of tissue CK19 and HBME-1 of group A and group B were significantly higher than those of group C,the positive expression of tissue CD56 and TPO of group A and group B were significantly lower than those of group C,the positive expression of group A was significantly lower than that of group B,the differences were statistically significant(P<0.05),the positive expression of tissue CD56,CK19,TPO,HBME-1 of group A and group B were compared,the differences were not statistically significant(P>0.05);the spearman correlation analysis showed the tissue CD56,CK19,TPO and HBME-1 had no correlation with the lymph node metastasis of patients with thyroid micropapillary carcinoma(P>0.05).Conclusion The expression of tissue CD56,CK19,TPO,HBME-1 of patients with thyroid micropapillary carcinoma are significant,and they have no close correlation with the lymph node metastasis,so they have certain detection value in thyroid micropapillary carcinoma.
