摘 要:目的:探讨血清细胞间粘附分子-1(ICAM-1)、克拉拉细胞蛋白16(CC16)对新生儿呼吸窘迫综合征(NRDS)的诊断价值。方法:选取2019年1月-2022年12月本院收治的NRDS新生儿98例(NRDS组),其中轻度37例、中度31例、重度30例,同期非肺病新生儿60例为对照组。均行血清ICAM-1、CC16水平检测并比较不同组别间差异,分析NRDS患儿血清ICAM-1、CC16水平与呼吸机参数吸入氧浓度(FiO_(2))和呼吸机使用天数的相关性,并评估对新生儿NRDS的诊断效能。结果:NRDS组血清ICAM-1(438.89±122.41μg/ml)、CC16(39.21±8.14 ng/ml)水平高于对照组(241.55±52.36μg/ml、11.17±3.69 ng/ml),且随着NRDS病情程度加重水平增高(均P<0.05)。Pearson相关分析显示,患儿血清ICAM-1、CC16水平与FiO_(2)水平和呼吸机使用天数均呈正相关(P<0.05);logistic回归分析,ICAM-1、CC16升高是NRDS发生的风险因素(P<0.05);ROC曲线分析,血清ICAM-1、CC16水平诊断新生儿NRDS的曲线下面积分别为0.791、0.850,二者联合检测诊断的曲线下面积(0.920)高于单一指标诊断(P<0.05)。结论:ICAM-1、CC16能够反映病情严重程度,可作为新生儿NRDS的诊断标志物。Objective:To investigate the diagnostic value of the serum intercellular adhesion molecule1(ICAM-1)and Clara cell protein 16(OC16)neonates for their respiratory distress syndrome(NRDS).Methods:A total of 98 neo-nates with NRDS were selected in study group from January 2019 to December 2022,including 37 cases with mild NRDS in group A.31 cases with moderate NRDS in group B,and 30 cases with severe NRDS in group C.And 60 neo-nates with non-pulmonary diseases were selected in control group during the same period.The serum ICAM-1 and CC16 levels of all the neonates were detected,and the differences of which of the neonates were compared among these groups.The correlation between the serum ICAM-1 and CC16 levels of the nconates and their ventilator parameters.such as inhaled oxygen concentration(Fi02),and their days of ventilator used was analyzed.The diagnostic eficacy of the levels of ICAM-1 and CC16 of the neonates for their NRDS was evaluated.Results:The levels of serum ICAM-1(438.89±122.41g/m1)and CC16(39.21±8.14 ng/ml)of the neonates in the study group were significantly higher than those(241.55±52.36pug/ml and 11.17±3.69 ng/ml)of the neonates in the control group,and which of the neo-nates in group A.group B,and group C had increased gradually(P<0.05).Pearson correlation analysis showed that the serum ICAM-1 and CC16 levels of the neonates in the study group were positively correlated with their FiO_(2) levels and their days of ventilator used(P<0.05).Logistic regression analysis showed that the levels of ICAM-1 and CC16 of the nconates in the study group were the independently correlative factors of the neonatal NRDS occurrence(P<0.05)..ROC curve analysis showed that the area under the curve of the serum ICAM-1 level or CC16 level of the neonates for diagnosing their NRDS was 0.791 or 0.850,and which was significantly lower than that(0.920)of the combined serum.ICAM-1 and CC16 levels(P<0.05).Conclusion:The serum ICAM-1 and CC16 levels of the neonates reflect the se-verity of their NRDS,and which can be use
