血府逐瘀汤加减联合西药治疗血瘀型继发性闭经的效果观察

摘　　要：目的:探讨血府逐瘀汤加减联合西药治疗血瘀型继发性闭经的临床效果。方法:选取2021年1—12月徐州丰县中医医院收治的84例血瘀型继发性闭经患者作为研究对象,采用随机数字表法分为中药组与西药组,各42例。西药组予以雌孕激素序贯治疗,中药组在西药组治疗基础上予以血府逐瘀汤加减治疗。比较两组临床疗效。结果:治疗后,两组月经停闭、小腹疼痛、肌肤甲错、口干不欲饮积分均低于治疗前,且中药组低于西药组,差异有统计学意义(P<0.001)。治疗后,两组血清卵泡刺激素、黄体生成素、睾酮、雌二醇水平低于治疗前,且中药组低于西药组,两组促抗缪勒管激素水平高于治疗前,且中药组高于西药组,差异有统计学意义(P<0.05)。中药组月经质量恢复时间及月经周期恢复时间短于西药组,不良反应发生率低于西药组,差异有统计学意义(P<0.05)。结论:血府逐瘀汤加减治疗血瘀型继发性闭经的效果显著,可有效减轻患者中医证候,改善内分泌情况,调节性激素水平,促进月经恢复,降低不良反应发生率。Objective:To investigate the clinical effect of modified Xuefu Zhuyu Decoction combined with western medicine in the treatment of secondary amenorrhea of blood stasis type.Methods:From January 2021 to December 2021,84 patients with secondary amenorrhea of blood stasis type were selected from Xuzhou Feng County Hospital of Traditional Chinese Medicine.They were divided into Chinese medicine group and western medicine group according to random number table method,with 42 cases in each group.The western medicine group was treated with estrogen and progesterone in sequence,and the Chinese medicine group was treated with modified Xuefu Zhuyu Decoction on the basis of the western medicine group.The clinical efficacy of the two groups was compared.Results:After treatment,the scores of amenorrhea,lower abdomen pain,squamous and dry skin,and dry mouth in two groups were lower than those before treatment,and the Chinese medicine group was lower than western medicine group,and the difference was statistically significant(P<0.001).After treatment,the serum levels of follicle-stimulating hormone,luteinizing hormone,testosterone and estradiol in the two groups were lower than those before treatment,and the Chinese medicine group was lower than the western medicine group,while the levels of anti-mullerian tube hormone in the two groups were higher than those before treatment,and the Chinese medicine group was higher than that the western medicine group,and the difference was statistically significant(P<0.05).The menstrual quality recovery time and menstrual cycle recovery time in the Chinese medicine group were shorter than those in the western medicine group,and the incidence of adverse reactions in the Chinese medicine group was lower than that in the western medicine group,and the difference was statistically significant(P<0.05).Conclusion:The modified Xuefu Zhuyu Decoction has a significant effect in the treatment of secondary amenorrhea of blood stasis type,it can effectively relieve TCM syndrome of patients,improve the en
