摘  要:目的探讨早期胰岛素治疗妊娠期糖尿病(GDM)的临床效果。方法选取90例GDM患者为研究对象,随机分为两组,各45例。对照组于晚期(孕32周后)开始使用胰岛素治疗,试验组于早期(孕32周前)开始使用胰岛素治疗,比较两组血糖指标、妊娠并发症、分娩结局及不良妊娠结局。结果试验组治疗后空腹血糖、餐后2h血糖水平低于对照组(P<0.05);试验组妊娠并发症发生率、早产率、剖宫产率、不良妊娠结局发生率分别为4.44%、4.44%、26.67%、4.44%,低于对照组的20.00%、22.22%、48.89%、24.44%(P<0.05)。结论GDM患者接受早期胰岛素治疗,能明显改善血糖指标,降低妊娠并发症、不良妊娠结局发生率,提高经阴道分娩率。Objective To explore the clinical effect of early insulin in the treatment of gestational diabetes mellitus(GDM).Methods90 patients with GDM were randomly divided into two groups,45 cases in each group.The control group began to use insulin treatment in the late stage(after 32 weeks of pregnancy),and the test group began to use insulin treatment in the early stage(before 32 weeks of pregnancy).The blood glucose indicators,complications of pregnancy,delivery outcomes and adverse pregnancy outcomes of the two groups were compared.Results After treatment,the levels of glucose fasting blood sugar and blood glucose 2 hours after meal in the test group were lower than those in the control group(P<0.05);The incidence of complications of pregnancy,premature delivery rate,cesarean section rate,and adverse pregnancy outcomes in the test group were 4.44%,4.44%,26.67%,and 4.44%,respectively,lower than 20.00%,22.22%,48.89%,and 24.44%in the control group(P<0.05).Conclusion Early insulin therapy for GDM patients can significantly improve blood glucose indicators,reduce the incidence of complications of pregnancy and adverse pregnancy outcomes,and increase the rate of vaginal delivery.
