超声检查诊断剖宫产后子宫瘢痕妊娠分型对临床选择治疗方案的价值

摘　　要：目的探讨超声检查诊断剖宫产后子宫瘢痕妊娠分型对临床选择治疗方案的价值。方法选取本院接收的50例剖宫产后子宫瘢痕妊娠患者为研究对象,均接受超声检查,分析其瘢痕类型,并选择合适的治疗方案。结果Ⅲ型患者的血供丰富发生率显著高于Ⅰ型、Ⅱ型,子宫前壁下段肌层厚度显著小于Ⅰ型、Ⅱ型,绒毛入侵肌层发生率显著低于Ⅰ型、Ⅱ型(P<0.05)。Ⅰ型患者68.00%实施了子宫动脉栓塞术,32.00%实施了宫腔镜下子宫瘢痕妊娠病灶剔除术;Ⅱ型患者50.00%实施了宫腔镜下子宫瘢痕妊娠病灶剔除术,50.00%实施了腹腔镜下子宫瘢痕妊娠病灶剔除术;Ⅲ型患者100%实施了腹腔镜下子宫瘢痕妊娠病灶剔除术。结论超声检查能有效诊断剖宫产后子宫瘢痕妊娠的瘢痕类型,为临床合理选择治疗方案提供参考。Objective To explore the value of ultrasonography in diagnosis of uterine scar pregnancy after cesarean section in clinical treatment options.Methods 50 cases of uterine scar pregnancy after cesarean section received by our hospital were selected as the research objects.All of them were examined by ultrasound,and their scar types were analyzed,and appropriate treatment plans were selected.Results The incidence of blood supply enrichment in typeⅢpatients was significantly higher than that in typeⅠandⅡ,the thickness of the lower segment of the anterior uterine wall muscle layer is significantly smaller than that of typeⅠandⅡ,the incidence of villus invasion into the muscle layer was significantly lower than that in typeⅠandⅡ(P<0.05).68.00%of TypeⅠpatients underwent uterine artery embolization,and 32.00%underwent hysteroscopic removal of uterine scar pregnancy lesions;50.00%of typeⅡpatients underwent hysteroscopic removal of uterine scar pregnancy lesions,and 50.00%underwent laparoscopic removal of uterine scar pregnancy lesions;100%of typeⅢpatients underwent laparoscopic removal of uterine scar pregnancy lesions.Conclusion Ultrasonography can effectively diagnose the type of uterine scar pregnancy after cesarean section,and provide reference for clinical rational choice of treatment scheme.
