恩格列净对老年2型糖尿病合并高血压患者血压变异性和左室质量指数的影响

摘　　要：目的观察恩格列净对老年2型糖尿病合并高血压患者血压变异性(BPV)和左室质量指数(LVMI)的影响。方法选取2022年2月—2023年2月福建省漳州市医院全科医学科收治的老年2型糖尿病合并高血压患者104例,采用随机抽样方式分为联合组和常规组,各52例。常规组给予基础降糖、降压药物治疗,联合组在常规组治疗基础上给予恩格列净治疗,2组患者均治疗3个月。比较2组治疗前后BPV相关指标、LVMI、左室射血分数(LVEF)与空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA_(1c)),以及不良反应。结果治疗3个月后,2组患者24 h、白昼、夜间收缩压变异性和舒张压变异性均低于治疗前,且联合组低于常规组(P<0.05或P<0.01);2组患者LVMI低于治疗前,LVEF高于治疗前,且联合组降低/升高幅度大于常规组(P均<0.01);2组患者FPG、2 hPG水平及HbA_(1c)均低于治疗前,且联合组低于常规组(P<0.01)。联合组不良反应总发生率为9.62%,低于常规组的25.00%(χ^(2)=4.300,P=0.038)。结论恩格列净治疗老年2型糖尿病合并高血压患者具有较好的临床效果,安全性高,值得临床推广应用。Objective To observe the effect of empagliflozin on blood pressure variability(BPV)and left ventricular mass index(LVMI)in elderly patients with type 2 diabetes mellitus complicated with hypertension.Methods A total of 104 elderly patients with type 2 diabetes mellitus complicated with hypertension admitted to the Department of General Medicine of Zhangzhou Municipal Hospital of Fujian Province from February 2022 to February 2023 were selected and randomly divided into joint group and regular group,52 cases in each group.The regular group was treated with basic hypoglycemic and antihypertensive drugs,and the joint group was treated with empagliflozin on the basis of the regular group,both groups were treated for 3 months.BPV related indexes,LVMI,left ventricular ejection fraction(LVEF),fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 hPG),hemoglobin A 1c(HbA_(1c))before and after treatment,and the incidence of adverse reactions were compared between the two groups.Results After 3 months of treatment,24 h,day and night systolic blood pressure variability diastolic blood pressure variability in the two groups were lower than those before treatment,and joint group was lower than regular group(P<0.05 or P<0.01);LVMI in the two groups was lower than those before treatment,LVEF was higher than that before treatment,and the decrease/increase of joint group was greater than regular group(P<0.01).The levels of FPG,2 hPG and HbA_(1c) in the two groups were lower than those before treatment,and the joint group was lower than regular group(P<0.01).The total incidence of adverse reactions in joint group was 9.62%,which was lower than 25.00%of regular group(χ^(2)=4.300,P=0.038).Conclusion In the treatment of elderly patients with type 2 diabetes mellitus complicated with hypertension,empagliflozin has good clinical effect,high safety,worthy of clinical application.
