六味地黄丸联合雌激素治疗围绝经期综合征的临床效果

摘　　要：目的观察六味地黄丸联合雌激素治疗围绝经期综合征的临床效果。方法选取2016年1月—2019年12月句容市中医院妇科门诊收治的围绝经期综合征患者112例,采用随机数字表法分为观察组和对照组,各56例。对照组予戊酸雌二醇片治疗,观察组在对照组基础上加用六味地黄丸治疗,2组均治疗3个月经周期。比较2组治疗效果,治疗前后中医证候积分与性激素[雌二醇(E_(2))、黄体生成素(LH)、卵泡刺激素(FSH)]水平变化,以及不良反应(胃肠不适、乳房胀痛等)。结果观察组患者治疗总有效率为96.43%,高于对照组的75.00%(χ^(2)=10.500,P=0.001);治疗后,2组潮热出汗、精神改变、眩晕头痛、感觉障碍积分以及LH、FSH水平均较治疗前降低,E_(2)水平较治疗前升高,且观察组降低或升高的程度均大于对照组(P均<0.01);观察组与对照组不良反应总发生率(1.79%vs.3.57%)比较差异无统计学意义(χ^(2)=0.343,P=0.558)。结论六味地黄丸联合雌激素治疗围绝经期综合征的临床效果显著,可有效改善患者性激素水平,缓解临床症状及中医证候群,提高生活质量,值得临床推广应用。Objective To observe the clinical effect of Liuwei Dihuang pills combined with estrogen in the treatment of perimenopausal syndrome.Methods A total of 112 patients with perimenopausal syndrome admitted to the gynecology clinic of Jurong Traditional Chinese Medicine Hospital from January 2016 to December 2019 were selected,and they were divided into observation group and control group used random number table method,with 56 cases in each group.The control group was treated with estradiol valerate tablets,while the observation group was treated with Liuwei Dihuang pills in addition to the control group.Both groups were treated for 3 menstrual cycles.Compared the treatment effect of two groups,and compared the changes in traditional Chinese medicine syndrome scores and levels of sex hormones[estradiol(E_(2)),luteinizing hormone(LH),follicle stimulating hormone(FSH)]before and after treatment,as well as the occurrence of adverse reactions(gastrointestinal discomfort,breast pain,etc.).Results The total effective rate of treatment in the observation group was 96.43%,higher than 75.00%in the control group(χ^(2)=10.500,P=0.001).After treatment,the scores of hot flashes,sweating,mental changes,dizziness,headache,sensory disorders,LH and FSH levels in the two groups decreased compared to before treatment,while E_(2) level increased compared to before treatment.The degree of decrease or increase in the observation group was greater than that in the control group(all P<0.01).There was no statistically significant difference in the total incidence of adverse reactions between the observation group and the control group(1.79%vs.3.57%,χ^(2)=0.343,P=0.558).Conclusion The clinical effect of Liuwei Dihuang pills combined with estrogen in the treatment of perimenopausal syndrome is significant,it can effectively improve patients′sex hormone levels,relieve clinical symptoms and TCM syndrome groups,and improve the quality of life.it is worthy of clinical application.
