屋尘螨变应原制剂治疗儿童变应性鼻炎的效果及对总IgE和Th17水平的影响

摘　　要：目的观察屋尘螨变应原制剂治疗儿童变应性鼻炎(AR)的临床效果及对总免疫球蛋白E(IgE)和Th17水平的影响。方法选取2019年12月—2021年2月南方医科大学深圳医院收治的AR患儿80例作为研究对象,根据随机数字表法分为研究组和对照组,每组40例。对照组患儿给予临床常规治疗,研究组患儿给予屋尘螨变应原制剂治疗,2组均治疗1年。比较2组患儿治疗效果,治疗前及治疗1年后血清总IgE、特异性免疫球蛋白、细胞因子水平及不良反应。结果研究组治疗总有效率为95.00%,高于对照组的77.50%(χ^(2)=5.165,P=0.023)。治疗1年后,2组总IgE、特异性免疫球蛋白E(sIgE)均低于治疗前,特异性免疫球蛋白G4(sIgG4)水平高于治疗前,且研究组降低/升高幅度大于对照组(P<0.05或P<0.01)。治疗1年后,2组血清Th17细胞因子白介素-17(IL-17)和Treg细胞因子白介素-10(IL-10)水平均低于治疗前,且研究组低于对照组(P<0.01)。研究组不良反应总发生率为2.50%,低于对照组的17.50%,但差异无统计学意义(χ^(2)=3.472,P=0.062)。结论屋尘螨变应原制剂治疗儿童AR的临床疗效确切,可有效提高患儿的免疫功能,减轻机体炎性反应,值得临床推广。Objective To observe the clinical effect of house dust mite allergen preparation on treating children with allergic rhinitis(AR)and its influence on the levels of total immunoglobulin E(IgE)and Th17.Methods Eighty children with AR admitted to Shenzhen Hospital of Southern Medical University from December 2019 to February 2021 were selected as the study subjects,and they were divided into study group and control group according to random number table method,with 40 cases in each group.The children in the control group were given routine clinical treatment,and the children in the study group were given house dust mite allergen preparation,both groups were treated for 1 year.The treatment efficacy,levels of serum total IgE,immunoglobulin,inflammatory factors and adverse reactions were compared between the two groups before and after treatment for one year.Results The total effective rate of treatment in the study group was 95.00%,higher than 77.50%in the control group(χ^(2)=5.165,P=0.023).After 1 year of treatment,the levels of total IgE,specific immunoglobulin E(sIgE)in the two groups were lower than those before treatment,and specific immunoglobulin G4(sIgG4)in the two groups was higher than that before treatment,the magnitude of decrease/increase in the study group were greater than those in the control group(P<0.05 or P<0.01).After 1 year of treatment,the levels of serum Th17 cytokine interleukin-17(IL-17)and Treg cytokine interleukin-10(IL-10)in the two groups were lower than those before treatment,and the levels in the study group were lower than those in the control group(P<0.01).The total incidence of adverse reactions in the study group was 2.50%,lower than 17.50%in the control group,but the difference was not statistically significant((χ^(2)=3.472,P=0.062)).Conclusion The clinical efficacy of house dust mite allergen preparation on treating children with AR is definite,which can effectively improve the immune function of children and reduce the inflammatory reaction of their body,and it is worthy of clin
