摘 要:目的 系统分析阿奇霉素(AZI)治疗移植后闭塞性细支气管炎(BO)的效果。方法 检索PubMed、Cochrane Central Register of Controlled Trials、Embase、International Trial Registers、中国生物医学文献数据库、中国知网、万方数据库及维普数据库,检索时间从建库至2019年6月29日。两名研究人员独立筛选、审查和提取研究数据。使用STATA V.12.0进行统计学分析。结果 共纳入7项研究,356例移植后BO患者在AZI治疗后随访3~4个月,AZI组与非AZI组的第1秒用力呼气容积差异无统计学意义(WMD=-0.08,95%CI-0.13~-0.03,P=0.128),异质性不显著(I^(2)=39.5%)。亚组分析后RCT组、LT组提示2组有显著性差异(WMD=-0.13,95%CI-0.26~0.01,P=0.009;WMD=-0.08,95%CI-0.13~-0.02,P=0.004)。结论 AZI可能会改善移植后BO患者的肺功能,但还需进一步的研究证实此结论。Objective To analyze the effects of azithromycin(AZI)in the treatment of post-transplant bronchiolitis obliterans(BO)syndrome.Methods Retrieved PubMed,Cochrane Central Register of Controlled Trials,Embase,International Trial Registers,China Biomedical Literature Database,China National Knowledge Infrastructure,Wanfang Database and VIP Database from establishment to June 29,2019.Two researchers independently screened,reviewed and extracted research data.Perform statistical analysis using STATA V.12.0.Results A total of 7 studies were included,and 356 post transplant BO patients were followed up for 3~4 months after AZI treatment,there was no significant difference in forced expiratory volume at the first second between the AZI group and the non AZI group(WMD=-0.08,95%CI-0.13~-0.03,P=0.128),and heterogeneity was not significant(I^(2)=39.5%).After subgroup analysis,the RCT group and LT group showed significant differences between the two groups(WMD=-0.13,95%CI-0.26~0.01,P=0.009;WMD=-0.08,95%CI-0.13~-0.02,P=0.004).Conclusion AZI can improve lung function in post transplant BO patients,but further research is needed to confirm this conclusion.
