摘 要:目的:探讨超声检测职业慢性苯中毒患者颈动脉及椎动脉的血流动力学变化。方法:选取2021年9月-2023年3月于东莞市第六人民医院确诊为职业慢性苯中毒的70例患者作为观察组,选择同期在我院体检健康者70名为对照组。均采用超声诊断仪对颈动脉和椎动脉进行检查,观察并记录两组动脉内径、颈动脉在收缩期峰值血流速度(peak velocity of systolic blood flow,Vs)、颈动脉血管舒张末期流速(end diastolic velocity,Vd)、颈动脉血管阻力指数(resistance index,RI)等相关超声参数。结果:两组两侧颈总动脉内径、颈内动脉内径对比差异无统计学意义(P> 0.05);观察组两侧颈总动脉、颈内动脉RI值均高于对照组(P <0.05);两组两侧颈总动脉、颈内动脉的Vs值差异无统计学意义(P> 0.05);观察组两侧颈总动脉、颈内动脉Vd值均低于对照组(P <0.05)。两组椎动脉内径、Vs值差异无统计学意义(P> 0.05);观察组椎动脉RI值显著高于对照组,Vd值显著低于对照组(P <0.01)。结论:超声检测可以对职业慢性苯中毒患者颈动脉及椎动脉血流动力学进行评估,对提高临床治疗职业慢性轻度苯中毒提供了较为可靠的依据。Objective To explore the hemodynamic changes of carotid artery and vertebral artery in occupational chronic benzene poisoning patients detected by ultrasonography.Methods A total of 70 patients diagnosed with occupational chronic benzene poisoning in the Sixth People's Hospital of Dongguan from September 2021 to March 2023 were selected as the observation group,and 70 healthy patients who underwent physical examination in our hospital during the same period were selected as the control group.Carotid artery and vertebral artery were examined by ultrasonic diagnostic instrument.Relevant ultrasonic parameters,such as internal diameter of arteries,peak velocity of systolic blood flow(Vs),end-diastolic velocity(Vd)and carotid vascular resistance index(RI),were observed and recorded in the two groups.Results There was no statistically significant difference in the diameter of the common carotid artery and internal carotid artery between the two groups(P>0.05);The RI values of both common and internal carotid arteries in the observation group were higher than those in the control group(P<0.05);There was no statistically significant difference in Vs values between the common carotid artery and internal carotid artery on both sides of the two groups(P>0.05);The Vd values of both common and internal carotid arteries in the observation group were lower than those in the control group(P<0.05).There was no statistically significant difference in vertebral artery diameter and Vs values between the two groups(P>0.05);The RI value of the vertebral artery in the observation group was significantly higher than that in the control group,while the Vd value was significantly lower than that in the control group(P<0.01).Conclusion Ultrasonography can evaluate the hemodynamics of carotid artery and vertebral artery in patients with occupational chronic benzene poisoning,and it provides a more reliable basis for improving the clinical treatment of occupational chronic mild benzene poisoning.
