获得性子宫动静脉畸形MRI诊断影像特征分析

摘　　要：目的:探讨获得性子宫动静脉畸形的临床表现和MRI特征性影像学表现,提高对其的认识及诊断准确率。方法:回顾性分析2011年6月—2022年6月经广州市荔湾中心医院确诊并治愈的9例子宫动静脉畸形患者的临床及MRI图像,并查阅近期国内外相关文献,总结子宫动静脉畸形的临床特点及MRI特征性表现。结果:子宫动静脉畸形MRI表现为子宫增大(8例)或正常(1例),肌层内斑片状或结节状异常信号,边界不清,无弥散受限,周围见迂曲血管流空信号影,增强扫描9例病变均呈明显不均匀强化,伴增粗的引流静脉影。结论:获得性子宫动静脉畸形MRI的特异性征象为子宫内多发迂曲血管流空信号影及引流静脉影,为临床诊断及治疗提供重要的参考价值。Objective Exploring the clinical manifestations of acquired uterine arteriovenous malformations and MRI characteristic imaging findings,is to improve our understanding on them for higher diagnostic accuracy.Methods The clinical and MRI images of 9 patients with uterine arteriovenous malformations diagnosed and cured by Liwan Central Hospital of Guangzhou were retrospectively analyzed,and relevant domestic and foreign literatures were consulted to summarize the clinical features and MRI characteristics of uterine arteriovenous malformations.Results The uterus was enlarged(8 cases)or normal(1 case),with patchy or nodular abnormal signals in the muscular layer,with unclear boundaries and no limitation of diffusion,the tortuous vessels were observed around.The enhanced scan lesions(9 cases)were obviously unevenly enhanced,and the thickened drainage vein shadow was observed.Conclusion The MRI characteristics of acquired uterine arteriovenous malformations are multiple tortuous vessel emptying signal shadows and draining vein shadows,which provide important reference value for clinical diagnosis and treatment.
