临床药师视角下甘精胰岛素U100转换应用甘精胰岛素U300致血糖波动1例

摘　　要：目的探讨胰岛素专项集采后患者胰岛素转换使用过程中出现的血糖波动,为加强胰岛素转换过程中的药物治疗管理提供参考。方法临床药师对1例甘精胰岛素U100转换应用甘精胰岛素U300后出现血糖波动的病例进行分析,探讨转换后血糖控制不佳的原因,并协同医生进行药物治疗调整。结果患者血糖恢复平稳,临床药师在患者胰岛素转换使用的安全性和有效性中发挥重要作用。结论胰岛素专项集采落地促进了胰岛素转换使用,临床药师应该将患者转换过程中的安全性和有效性作为监护的重点。Objective To explore the blood sugar fluctuations in the insulin conversion process of patients after the special collection of insulin,and provide a reference for strengthening the drug treatment management during the insulin conversion process.Methods Clinical pharmacists analyzed a case of blood sugar fluctuations after switching from insulin glargine U100 to insulin glargine U300,explored the reasons for poor blood sugar control after switching,and cooperated with doctors to give advice on drug treatment adjustments.Finally,the patient′s blood sugar stayed stable.Results Clinical pharmacists play an important role in the safety and efficacy of insulin switching in patients.Conclusion The implementation of the special collection of insulin has promoted the use of insulin conversion,and clinical pharmacists should focus on the safety and effectiveness of patients during the conversion process.
