摘　　要：目的探讨白细胞分化抗原38(cluster of differentiation 38,CD38)和白细胞分化抗原138(CD138)在慢性子宫内膜炎不孕不育症中的临床诊断价值。方法选取九江市妇幼保健院2018年4月—2021年10月收治的不孕不育症患者80例,将苏木精-伊红染色阳性患者纳入观察组,阴性患者纳入对照组,每组40例,对比2组子宫内膜CD38和CD138水平表达,对比抗感染治疗前后观察组的子宫内膜厚度、受孕情况及受孕者CD38、CD138水平表达。结果观察组CD38、CD138及联合检测的阳性率均高于对照组,差异有统计学意义(P<0.05)。观察组治疗14 d后子宫内膜厚度大于治疗前,差异有统计学意义(P<0.05)。观察组有38人受孕成功,受孕率高达95.00%,且受孕者CD38联合CD138检测均为阴性。结论检测子宫内膜中CD38、CD138表达可辅助诊断慢性子宫内膜炎不孕症,为该病治疗提供新的思路,具有临床参考价值。Objective To study the effect of nifedipine(Nif)combined with lowmolecular weight heparin calcium in the treatment of severe preeclampsia(severe preeclampsia,SPE)and its effect on coagulation function.Methods Using the random number table method,98 patients with SPE who were delivered by cesarean section in the obstetrics department of Huizhou First Maternal and Child Health Hospital from February 2019 to February 2021 were divided into the control group and the combined group,with 49 cases in each group.The control group was treated with NIF,and the combined group was treated with NIF combined with lowmolecular weight heparin calcium.The therapeutic effect,blood pressure,coagulation function and adverse pregnancy outcome were compared between the two groups.Results The effective rate of the combined group(97.96%)was higher than that of the control group(85.71%)(P<0.05).There was no statistical difference in systolic blood pressure(SBP)and diastole pressure(DBP)between the two groups before treatment(P>0.05)After treatment,the SBP and DBP of the two groups were lower than those before treatment,and the combination group was lower than the control group(P<0.05).There was no difference in FIB,PT,APTT and D-D between the two groups before treatment(P>0.05).Statistical significance,fibrinogen(FIB)and D-dimer(D-D)in the combined group after treatment were lower than those in the control group,prothrombin time(PT),activated partial thromboplastin time(APTT)was longer than that in the control group(P<0.05).The incidence of adverse pregnancy outcomes in the combined group(4.08%)was lower than that in the control group(18.36%)(P<0.05).Conclusion NIF combined with low molecular weight heparin calcium is effective in the treatment of SPE.It can reduce blood pressure,improve coagulation function and reduce adverse pregnancy outcomes.It is worthy of application.
