摘  要:目的通过网状Meta分析法系统评估和比较有氧运动、抗阻运动、间歇运动、有氧联合抗阻运动和身心运动对改善原发性高血压(EHT)患者血压和血脂水平的疗效,为EHT患者改善血压和血脂水平提供循证依据。方法系统检索PubMed、Cochrane、Embase、Web of Science、EBSCO和CBM数据库中关于运动改善EHT患者血压和血脂的随机对照试验。采用STATA16.0和Rev Man 5.3软件对纳入研究的数据进行Meta分析。结果共有45项涉及3560名患者的随机对照试验被纳入网状Meta分析。结果显示有氧联合抗阻运动对降低收缩压的效果最优(MD=-13.58,95%CI[-17.72,-9.44],p<0.05);身心运动对降低舒张压(MD=-8.61,95%CI[-10.52,-6.70],p<0.05)、甘油三酯(SMD=-0.98,95%CI[-1.73,-0.23],p<0.05)和低密度脂蛋白(SMD=-1.07,95%CI[-1.57,-0.56],p<0.05)的效果最优;有氧运动对降低总胆固醇(SMD=-0.52,95%CI[-0.79,-0.25],p<0.05)的效果最优。累积概率排序结果(SUCRA)与网状Meta分析结果一致。结论身心运动、有氧联合抗阻运动和有氧运动是改善原发性高血压患者血压和血脂水平的最佳运动方式。Objective The efficacy of aerobic exercise,resistance exercise,interval exercise,aerobic combined resistance exercise,and mind-body exercise in improving blood pressure and lipid levels in patients with essential hypertension(EHT)was systematically evaluated and compared by network meta-analysis,which provided an evidence-based basis for improving blood pressure and lipid levels in patients with EHT.Methods Randomised controlled trials in PubMed,Cochrane,Embase,Web of Science,EBSCO and CBM databases on exercise improving blood pressure and lipids in people with EHT.Data from included studies were meta-analysed using STATA16.0 and RevMan 5.3 software.Results A total of 45 randomised controlled trials involving 3560 patients were included in the network meta-analysis.The results showed that aerobic combined resistance exercise had the best effect on reducing systolic blood pressure(MD=-13.58,95%CI[-17.72,-9.44],p<0.05),and mind-body exercise had the best effect on reducing diastolic blood pressure(MD=-8.61,95%CI[-10.52,-6.70],p<0.05),triglycerides(SMD=-0.98,95%CI[-1.73,-0.23],p<0.05)and low-density lipoprotein(SMD=-1.07,95%CI[-1.57,-0.56],p<0.05),and aerobic exercise had the best effect on reducing total cholesterol(SMD=-0.52,95%CI[-0.79,-0.25],p<0.05).The cumulative probability ranking results(SUCRA)were consistent with the results of the network meta-analysis.Conclusion Mind-body exercise,combined aerobic and resistance exercise,and aerobic exercise are the optimal forms of physical activity for improving blood pressure and lipid levels in patients with EHT.
