摘　　要：非酒精性脂肪性肝炎发病率日益增高,已成为全球共同关注的健康问题。脂毒性是非酒精性脂肪性肝炎的重要发病环节,其既可激活巨噬细胞向促炎表型极化,产生肝脏炎症,还可诱导肝细胞线粒体损伤,介导肝细胞凋亡,并对器官或系统产生毒性损伤。中医学认为痰瘀互结是非酒精性脂肪性肝炎的重要病机之一,临床以化痰祛瘀为主要治法。在非酒精性脂肪性肝炎中,痰浊瘀血致病与脂毒性发病在病因病机、临床表现、疾病进展、生物学基础、治则治法等方面具有相似性,但两者之间的关系无明确实验证据,故从痰瘀理论角度阐释非酒精性脂肪性肝炎痰瘀互结证候及与脂毒性的关系,以期为今后研究和临床治疗提供理论依据。The increasing incidence of non-alcoholic steatohepatitis has become a global health concern.Lipotoxicity is an important pathogenesis of non-alcoholic steatohepatitis,which can activate the polarization of macrophages towards pro-inflammatory phenotype and produce liver inflammation.It can also induce mitochondrial damage of hepatocytes,mediate apoptosis of hepatocytes,and cause toxic damage to organs or systems.Phlegm-stasis interjunction is one of the important pathogenesis of non-alcoholic steatohepatitis and the main clinical treatment is to remove phlegm and blood stasis.In non-alcoholic steatohepatitis,the etiology,pathogenesis,clinical manifestations,disease progression,biological basis,treatment methods,and other aspects of phlegm-stasis and blood stasis are similar,but there is no clear experimental evidence for the relationship between the two.Therefore,from the perspective of phlegm-stasis theory,the syndrome of“phlegm-stasis interjunction”in non-alcoholic steatohepatitis and its relationship with lipid toxicity are expounded.The aim is to provide a theoretical basis for later research and clinical treatment.
