摘 要:目的探讨中国中老年人群血清尿酸/肌酐比值(SUA/Cr)与代谢综合征(MS)之间的相关性,为中老年人MS防治提供科学依据。方法数据来源于2015年中国健康与养老追踪调查,选择其中完成SUA测定的45岁及以上居民为研究对象,收集一般人口学信息、行为生活方式、体格检查、生理健康状况及血液生化检查等相关数据。根据SUA/Cr水平的四分位数将研究对象分为Q_(1)~Q_(4)组4组,采用SPSS 25.0软件进行t检验、秩和检验、χ^(2)检验、趋势χ^(2)检验和方差分析,采用多因素logistic回归模型分析SUA/Cr水平与MS及其组分的相关性。结果纳入研究对象9202名,平均年龄为(59.8±9.6)岁,其中男性4281名,女性4921名,MS检出率为24.1%,SUA/Cr水平为4.28±1.17。MS组年龄、高尿酸血症检出率、高血压史比例、糖尿病史比例、血脂异常史比例、糖化血红蛋白、收缩压(SBP)、舒张压(DBP)、体质指数(BMI)、腰围、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FPG)、C反应蛋白、SUA、SUA/Cr均高于非MS组,高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、肾小球滤过率(eGFR)均低于非MS组,差异均有统计学意义(P<0.01)。Q_(1)~Q_(4)组MS检出率分别为14.7%、20.0%、24.2%和37.5%。调整混杂因素后,多因素logistic回归分析结果显示,与Q1组相比,男性Q_(4)组(OR=1.987,95%CI:1.477~2.672)和女性Q_(4)组(OR=2.563,95%CI:1.971~3.333)MS患病风险均显著升高;且Q_(2)~Q_(4)组MS组分中中心性肥胖、高TG血症和低HDL-C血症患病风险也均显著升高,均有统计学意义(P<0.01)。结论中国中老年人群SUA/Cr水平与MS呈正向关联,SUA/Cr与MS及其组分密切相关,高SUA/Cr水平可能是中老年人MS发生的危险因素。Objective To explore the correlation between serum uric acid to creatinine ratio(SUA/Cr)and metabolic syndrome(MS)in middle-aged and elderly people of China,and provide the scientific evidence for the prevention and treatment of MS in middle-aged and elderly people.Methods The data were from the China Health and Retirement Longitudinal Study(CHARLS)in 2015.Total 9202 valid cases(≥45 years old)with blood test(SUA)were selected as the objects.Data of general demographic information,behavior and lifestyle,physical examination,physical health status,blood biochemical examination and other relevant data were collected.According to the quartiles of SUA/Cr levels,the subjects were divided into Q_(1)-Q_(4)groups,The t test,rank sum test,χ^(2)test,trend χ^(2)test and ANOVA were used to analyze the data.The correlation between SUA/Cr and MS and its components were analyzed by multivariate logistic regression model.The used software was SPSS 25.0.Results A total of 9202 subjects(male:4281 cases;female:4921 cases)were included,the average age was(59.8±9.6)years old.The detection rate of MS was24.1%,and the level of SUA/Cr was 4.28±1.17.The age,the detected rate of hyperuricemia,the proportions of hypertension,diabetes and dyslipidemia history,glycated hemoglobin(HbA_(1C)),systolic blood pressure(SBP),diastolic blood pressure(DBP),body mass index(BMI),waist circumference(WC),triglyceride(TG),total cholesterol(TC),fasting blood glucose(FPG),hypersensitive C-reactive protein(hs-CRP),serum uric acid(SUA),SUA/Cr ratio in MS group were significantly higher than those in non-MS group,while high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)and glomerular filtration rate(eGFR)in MS group were significantly lower than those in non-MS group(P<0.01).The detection rates of MS in the Q_(1)-Q_(4)groups were 14.7%,20.0%,24.2% and 37.5%,respectively.After adjusting the confounding factors,the results of multivariate logistic regression model showed that as compared with group Q_(1),the MS risk in mal
