摘 要:目的探讨瓜氨酸在增生型糖尿病视网膜病变(PDR)患者房水中的浓度变化及其与炎症因子表达的相关性。方法选取拟行玻璃体切割术的PDR患者24例纳入研究组,选择同期因年龄相关性白内障拟行超声乳化白内障吸除术的无糖尿病史的患者24例纳入对照组。使用高效液相色谱串联质谱法检测患者房水样本中的瓜氨酸浓度,采用多重微珠免疫法检测患者房水中白细胞介素(IL)-6、IL-8、IL-10、IL-1β、IL12-P70、IL-17A含量。采用Pearson相关分析确定瓜氨酸浓度与炎症因子含量间的相关性。结果研究组和对照组患者房水中瓜氨酸浓度分别为(17.946±11.354)μmol·L^(-1)和(28.612±7.135)μmol·L^(-1),两组间差异有统计学意义(t=3.896,P<0.001)。研究组患者房水中IL-6、IL-8、IL-1β含量均高于对照组,差异均有统计学意义(均为P<0.05)。Pearson相关分析结果显示:研究组患者房水中瓜氨酸浓度与IL-8含量呈负相关性(r=-0.609,P=0.036),而与其他炎症因子含量无明显相关性(均为P>0.05)。结论PDR患者房水中瓜氨酸浓度下降,并与IL-8含量呈负相关性,提示瓜氨酸表达失调可能参与了DR的发生发展过程。Objective To investigate the concentration change of citrulline in the aqueous humor(AH)of patients with proliferative diabetic retinopathy(PDR)and its correlation with the expression of inflammatory factors.Methods Totally,24 patients with PDR who were scheduled to undergo vitrectomy were included in the study group,and 24 patients with age-related cataract and without diabetes who were scheduled to undergo phacoemulsification during the same period were included in the control group.Citrulline concentration in AH samples was measured by high-performance liquid chromatography-mass spectrometry,and the levels of interleukin(IL)-6,IL-8,IL-10,IL-1β,IL12-P70 and IL-17A in AH were measured by multiplex bead immunoassay.The correlation between the concentration of citrulline and the level of inflammatory factors was determined through Pearson correlation analysis.Results The citrulline concentration in the study and control groups was(17.946±11.354)μmol·L^(-1) and(28.612±7.135)μmol·L^(-1),respectively,and the difference between the two groups was statistically significant(t=3.896,P<0.001).The levels of IL-6,IL-8 and IL-1β in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Pearson correlation analysis results showed that the citrulline concentration in the study group was negatively correlated with the IL-8 level(r=-0.609,P=0.036),but had no significant correlation with the levels of the other inflammatory factors(all P>0.05).Conclusion The concentration of citrulline in the AH of PDR patients decreases and is negatively correlated with the level of IL-8,suggesting that the dysregulation of citrulline expression may be involved in the occurrence and development of DR.
