基于预警分级理念儿童血液净化血流动力学稳定性护理方案的构建与应用

摘　　要：目的构建基于预警分级理念的儿童血液净化血流动力学稳定性护理方案,并分析其临床应用效果.方法采用便利抽样法,将行血液净化治疗的166例患儿分为对照组(n=87)和试验组(n=79);对照组采用常规护理,试验组在对照组的基础上采用基于预警分级理念的血流动力学稳定性护理;比较两组血液净化治疗血流动力学稳定性(低血压、低中心静脉压、下腔静脉血管直径变窄)发生率、管路使用时长、护士工作量及患儿住院费用.结果试验组低血压、低中心静脉压、下腔静脉血管直径变窄发生率均低于对照组(P<0.05);试验组管路使用时长高于对照组(P<0.05),护士工作量及患儿住院费用均低于对照组(P<0.05).结论应用基于预警分级理念的儿童血液净化血流动力学稳定性护理方案可降低患儿血流动力学不稳定性发生率,提高治疗连续性及护理安全性.Objective Construct the hemodynamic stability nursing plan for children’s blood purification based on the concept of early warning grading,and analyze its application effect.Method 166 children who received blood purification treatment were divided into control group(n=87)and experimental group(n=79)by using convenience sampling method.The experimental group received hemodynamic stability nursing based on the concept of early warning classification,and the control group received routine nursing.The incidence of hemodynamic stability(hypotension,low central venous pressure,narrowing of inferior vena cava vessel diameter),duration of pipeline use,workload of nurses and hospitalization costs of the two groups were compared.Results The incidence of hypotension,central venous pressure and inferior vena cava vessel diameter narrowing in the experimental group was lower than those in the control group(P<0.05).The length of tube use in the experimental group was higher than that in the control group(P<0.05),and the workload of nurses and hospitalization costs of children were lower than thost in the control group(P<0.05).Conclusion The application of hemodynamic stability nursing program of children blood purification based on the concept of early warning grading can reduce the incidence of hemodynamic instability,improve the continuity of treatment and nursing safety.
