自拟中药化脂方治疗代谢相关脂肪性肝病合并糖尿病前期的效果评价

摘　　要：目的探讨中药自拟化脂方治疗代谢相关脂肪性肝病(MAFLD)合并糖尿病前期的临床效果。方法将80例溆浦县中医医院2021年3—12月收治的MAFLD合并糖尿病前期患者依据随机数表法分成观察组和对照组,每组各40例,两组均常规给予生活方式干预,观察组在对照组的基础上加用中药自拟化脂方治疗,疗程均为8周。比较两组患者的肝脂肪衰减指数(CAP)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)。结果治疗后观察组CAP显著低于对照组(P<0.05)。两组FPG、2 hPG治疗后明显降低(P<0.05),与对照组比较,观察组FPG明显更低(P<0.05),2 hPG比较差异无统计学意义(P>0.05)。观察组患者治疗后的肝功能指标包括天门冬氨酸氨基转移酶(40.07±6.31)U/L、谷氨酸氨基转移酶(35.21±4.32)U/L、白蛋白(38.94±2.95)g/L、总胆红素(18.04±2.52)μmol/L改善程度显著优于对照组(P<0.05)。结论中药自拟化脂方应用于MAFLD合并糖尿病前期患者治疗中,可改善脂肪肝及血糖水平,提升患者肝功能,值得临床借鉴。Objective To investigate the clinical efficacy of self-formulated Chinese materia medica greaseeliminating prescription in the treatment of metabolic-associated fatty liver disease(MAFLD)combined with prediabetes.Methods A total of 80 patients with MAFLD combined with prediabetes admitted to and treated in Xupu Hospital of Traditional Chinese Medicine from March to December 2021 were divided into the observation group(n=40)and the control group(n=40)according to the random number table method.Both groups were treated with conventional lifestyle intervention.The observation group was additionally treated with self-formulated Chinese materia medica grease-eliminating prescription on the basis of the treatment in the control group.The treatment courses for both groups were 8 weeks.The controlled attenuation parameter(CAP)of liver fat,fasting plasma glucose(FPG),and 2-hour postprandial blood glucose(2 hPG)of patients in the two groups were compared.Results After treatment,the CAP of the observation group was significantly lower than that of the control group(P<0.05).After treatment,the FPG and 2 hPG of the two groups showed a significant decrease(P<0.05),and compared with the control group,the FPG of the observation group was significantly lower(P<0.05).There was no statistically significant difference between the two groups in 2 hPG(P>0.05).After treatment,the improvement in hepatic function indices of patients in the observation group,including aspartate aminotransferase(40.07±6.31)U/L,alanine aminotransferase(35.21±4.32)U/L,albumin(38.94±2.95)g/L,and total bilirubin(18.04±2.52)µmol/L,was significantly superior to the control group(P<0.05).Conclusion The application of self-formulated Chinese materia medica grease-eliminating prescription in the treatment of patients with MAFLD combined with prediabetes can alleviate the levels of fatty liver and blood glucose,and enhance the hepatic function of patients.Therefore,it is worthy of clinical reference.
