摘 要:目的:探讨3.0 T MRI三维多回波恢复梯度回波序列(3D_MERGE)与肩关节常规序列肩袖图像质量间的差异。方法:2019年6月至2019年8月于昆山市中医医院招募40名健康志愿者完成肩关节MRI常规序列及3D_MERGE序列成像。3D_MERGE序列于冠状位采集并作与常规序列同扫描方位(轴位、斜冠位、斜矢位)和肩袖肌腱长轴位的后处理重建。两观察者分别对两种序列所获图像进行图像质量主观评分;比较各年龄组两观察者对两种序列图像质量的主观评分间的差异和两观察者图像质量主观评分间的一致性。分析两种序列图像质量主观评分均值间的差异;分析3D_MERGE序列图像在肩袖各组成肌腱[冈上肌肌腱(SST)、冈下肌肌腱(IST)、肩胛下肌肌腱(SCT)和小圆肌肌腱(TMT)]图像质量主观评分均值间的差异。图像质量的客观评价以SST为参考,计算信噪比(SNR)和对比噪声比(CNR)并进行比较分析。结果:(1)两观察者对3D_MERGE序列和常规序列图像质量主观评分在各年龄组间差异均无统计学意义(t=-0.894~0.284,均P>0.05);(2)两观察者一致性比较:3D_MERGE序列和常规序列一致性均强(κ值均>0.80,均P<0.05);(3)3D_MERGE序列图像质量主观评分的均值均高于常规序列(均P<0.05);(4)3D_MERGE序列肩袖图像质量主观评分比较:(1)SST,长轴位和斜冠位高于轴位和斜矢位(均P<0.05);(2)IST,斜矢位高于轴位和斜冠位(均P<0.05);(3)SCT,长轴位、斜矢位和轴位均高于斜冠位(均P<0.05);(4)TMT,长轴位、斜矢位、轴位和斜冠位间差异无统计学意义(P=0.140);(5)3D_MERGE序列肩袖肌腱SNR及相关CNR均高于常规序列的对应值(均P<0.05)。结论:3D_MERGE序列肩袖图像质量评价在两观察者间的一致性强,主、客观图像质量均优于常规序列,可用于肩袖MR成像。Purpose:To investigate the difference in rotator cuff image quality between three-dimensional multi-echo recalled gradient echo sequence(3D_MERGE)and conventional sequence on 3.0 T MRI.Methods:From June 2019 to August 2019,about 40 healthy volunteers were recruited from Kunshan Traditional Chinese Medicine Hospital.They had undergone shoulder MRI with conventional sequence and 3D_MERGE sequence.The 3D_MERGE sequence acquisition was performed in coronal position and reconstructed in the same scanning orientation(axial,oblique coronal,oblique sagittal)as the conventional sequence and in the long axial position of rotator cuff tendon.Two observers scored the images obtained from 3D_MERGE sequence and conventional sequence subjectively,the differences of the subjective scores between the two observers on the image quality of both sequences in each age group were compared,and the consistency between the two observers was also conducted.The difference between the mean subjective scores on image quality of both sequences,and the difference between the mean subjective scores of image quality of each component tendon[supraspinatus tendon(SST),infraspinatus tendon(IST),subscapularis tendon(SCT)and teres minor tendon(TMT)]of the rotator cuff in 3D_MERGE sequence was compared.The objective evaluation of rotator cuff tendon image quality was represented by SST,and the signal to noise ratio(SNR)and contrast to noise ratio(CNR)were calculated and compared.Results:(1)There was no significant difference between the two observers in the subjective scores of 3D_MERGE sequence and conventional sequence image quality between different age groups(t=-0.894-0.284,all P>0.05).(2)Comparison of consistency between two observers:3D_MERGE sequence and conventional sequence both had strong consistency(both,κ>0.80,P<0.05).(3)The mean value of subjective scores of 3D_MERGE sequence image quality was higher than that of conventional sequence,and the difference was with statistical significance(all P<0.05).(4)Comparison of subjective scores of 3
