摘　　要：血管性痴呆(VaD)以脑血流量下降、能量代谢降低、认知功能减退为病理特点,长久以来对其病机存在不同争论,给中医科研带来困扰。营卫学说起源于《黄帝内经》,营卫协调是血脉运行通畅、腠理开阖有度、睡眠觉醒规律的重要条件。营卫学说在脑病领域内的拓展包括昼夜节律、认知学习、神经血管单元3个方面。营冱卫除是VaD的基本病机,应采用通调营卫的治法。营卫学说强调精神与气血的相互调控,具有整体性、动态性、双向性的特点,与玄府学说产生更广泛的理论内涵联系。将营卫学说引入VaD的病机认识和防治策略,不仅有助于加强对生命整体规律的把握,也能为中医相关科研工作提供新的思路。Vascular dementia is characterized by decreased cerebral blood flow,decreased energy metabolism and decreased cognitive function.There have been different debates on its pathogenesis for a long time,which has troubled the scientific research of traditional Chinese medicine.Ying-wei theory originated from Huangdi Neijing.The coordination of ying-wei is an important condition for smooth blood circulation,reasonable opening and closing,and the law of sleep and awakening.The expansion of the ying-wei theory in the field of encephalopathy includes circadian rhythm,cognitive learning and neurovascular unit.‘block ying and enervate wei’is the basic pathogenesis of vascular dementia,and the treatment of general regulation of‘unblock ying and harmonize wei’should be adopted.The ying-wei theory emphasizes the mutual regulation of spirit and qi and blood,which has the characteristics of integrity,dynamics and two-way,and has a broader theoretical connotation with Xuan Fu theory.The introduction of the ying-wei theory into the pathogenesis understanding and prevention strategy of vascular dementia not only helps to strengthen the grasp of the overall law of life,but also provides new ideas for the relevant scientific research of traditional Chinese medicine.
