血细胞参数WBC、LYM%、MO%、NE%、EOS%与甲型H1N1流感细胞免疫水平的相关性分析

摘　　要：目的分析血细胞参数WBC、LYM%、MO%、NE%、EOS%与甲型H1N1流感细胞免疫水平的相关性。方法本文选取2019年4月-2020年4月在我院治疗的甲型H1N1流感患者150例作为流感组,并选取在我院行健康体检的150例正常组作为对比,采用全自动血细胞分析仪检测血细胞参数白细胞计数(leucocyte count,WBC)、淋巴细胞比率(lymphocyte ratio,LYM%)、单核细胞比率(monocyte ratio,MO%)、中性粒细胞比率(neutrophil ratio,NE%)、嗜酸性粒细胞比率(eosin ratio,EOS%);采用免疫荧光定量检测法检测总T细胞、T辅助/T诱导细胞(T assisted/T induced cells,CD4)、T抑制/T杀伤细胞(T inhibition/T killer cells,CD8)、CD4/CD8在甲型H1N1流感患者中的表达及相关性。结果与正常组相比,流感组WBC、MO%、NE%、CD8水平均显著升高,差异具有统计学意义(P<0.05);与正常组相比,流感组LYM%、EOS%、总T细胞、CD4、CD4/CD8水平均显著降低,差异具有统计学意义(P<0.05)。与预后不良组相比,预后良好组血细胞参数WBC、MO%、NE%和CD8均显著降低,LYM%、EOS%、CD4、CD4/CD8和总T细胞均显著升高,差异具有统计学意义(P<0.05)。结论WBC、MO%、NE%、CD8在甲型H1N1流感中异常高表达,LYM%、EOS%、总T细胞、CD4、CD4/CD8在甲型H1N1流感中异常低表达,可作为临床上对甲型H1N1流感患者严重程度和预后的判断标准。Objective To analyze the correlation between blood cell parameters WBC,LYM%,MO%,NE%,EOS%,and cell immunity level of influenza A(H1N1).Methods 150 patients with influenza A H1N1 who were treated in our hospital from April 2019 to April 2020 were selected as the influenza group,and 150 normal patients who underwent physical examination in our hospital were selected as the comparison.Automatic blood cell analyzer was used to test leucocyte count(WBC),lymphocyte ratio(LYM%),and monocyte ratio(MO%),neutrophil ratio(NE%),eosin ratio(EOS%);Immunofluorescence assay was used to detect the expression and correlation of total T cells,T assisted/T induced cells(CD4),T inhibition/T killer cells(CD8),and CD4/CD8 in patients with H1N1.Results Compared with the normal group,the levels of WBC,MO%,NE%,and CD8 in the influenza group were significantly increased,with statistical significance(P<0.05).Compared with the normal group,the levels of LYM%,EOS%,total T cells,CD4,and CD4/CD8 in the influenza group were significantly decreased,and the differences were statistically significant(P<0.05).Compared with the poor prognosis,the blood cell parameters WBC,MO%,NE%and CD8 of good prognosis were significantly reduced,and LYM%,EOS%,CD4,CD4/CD8 and total T cells of good prognosis were significantly increased(P<0.05).Conclusion WBC,MO%,NE%,and CD8 are abnormally high expressed in influenza A(H1N1),and LYM%,EOS%,total T cells,CD4,CD4/CD8 were abnormally low expressed in influenza A(H1N1),which can be used as the criteria for the severity and prognosis of patients with influenza A(H1N1).
