摘  要:目的:探讨术中实时超声(IUS)在自发性脑出血(sICH)患者开颅手术中的治疗效果及应用价值。方法:回顾性分析2016年1月1日~2021年4月30日在皖南医学院第二附属医院神经外科住院手术治疗的38例sICH患者,根据术中是否IUS辅助分为超声辅助组和对照组,对比两组术中出血量、手术时间、血肿清除有效率、住院时间、术后90天格拉斯哥预后量表(GOS)评分等,评价IUS辅助治疗的效果。结果:与对照组相比,超声辅助组患者术中出血量少,手术时间及住院时间短,术后90天GOS评分高,差异均有统计学意义(P<0.05);两组患者血肿清除有效率差异无统计学意义(P>0.05)。结论:IUS辅助手术治疗sICH患者可有效缩短手术时间,减少出血量,改善患者神经功能预后,具有较高的临床应用价值。Objective:To investigate the therapeutic effect and application value of intraoperative real-time ultrasound in craniotomy for patients with spontaneous intracerebral hemorrhage(sICH).Methods:A retrospective analysis was performed in 38 sICH patients,admitted to and treated our hospital between January 1,2016 and April 30,2021.The patients were divided into ultrasound-assisted group and control group,according to whether intraoperative real-time ultrasound was used in surgery.Comparison was made between two groups in intraoperative blood loss,operative time,hematoma removal rate,hospital stay and Glasgow outcome scale(GOS)score 90 days after operation,in order to evaluate the effectiveness of real-time ultrasound assisted treatment.Results:Compared with the control group,patients in the ultrasound-assisted group had less intraoperative blood loss,shorter operative time and hospital stay,and higher GOS score.The differences were statistically significant(P<0.05).There was no significant difference in hematoma removal rate between two groups(P>0.05).Conclusion:Intraoperative real-time ultrasound assisted surgery for sICH patients can effectively shorten the operative time,reduce intraoperative blood loss and improve the prognosis of patients′neurological function.It has high clinical application value.
