摘  要:目的:探讨超声造影技术在周围型肺病变良恶性鉴别诊断中的临床应用价值。方法:收集安庆市立医院2020年9月~2022年5月经胸部CT诊断为周围型肺病变39例患者进行超声造影,记录超声造影剂在病灶的起始增强时间(AT)、达峰时间(TTP)、峰值强度(PI)、周边无病变肺组织起始增强时间(L-AT)等超声造影定量参数,以超声引导下穿刺活检或手术后病理诊断良恶性,分析以上超声造影定量参数在周围型肺病变良恶性鉴别诊断中的临床应用价值。结果:恶性病灶AT及△T指标均高于良性病灶,差异有统计学意义(P<0.05),两者在受试者工作特征(ROC)曲线下面积(AUC)分别为0.908、0.904,截点值分别为11.5、4.5 s,诊断灵敏度分别为65.4%、88.5%,特异度分别为100.0%、76.9%。结论:超声造影技术对周围型肺病变良恶性鉴别诊断具有较高的价值。Objective:To investigate the clinical value of contrast-enhanced ultrasound(CEUS)in the diagnosis of benign and malignant peripheral pulmonary lesions.Methods:The study included 39 patients,who were diagnosed with peripheral lung lesions by chest CT and underwent contrast-enhanced ultrasound in our hospital between September 2020 and May 2022.The data of arrival time(AT),time to peak(TTP),peak intensity(PI),lesion arrival time(L-AT)were recorded.Compared with the results of ultrasound-guided puncture biopsy or postoperative pathological examination,these indexes were used to analyze the clinical application value of CEUS in the diagnosis of benign and malignant peripheral pulmonary lesions.Results:The AT and △T of malignant lesions were significantly higher than those of benign lesions(P<0.05).The areas under the ROC curve of two indexes were 0.908 and 0.904,the cutoff values were 11.5 s and 4.5 s,the diagnostic sensitivity was 65.4%and 88.5%,and the specificity was 100.0%and 76.9%respectively.Conclusion:CEUS has a high value in diagnosis of benign and malignant peripheral pulmonary lesions.
