摘  要:目的:分析高原不同海拔低氧环境对女性卵巢储备功能与生育力的影响。方法:选择2019年5月—2021年1月就诊于我院生殖中心门诊患者,其中居住在玉树州、果洛州地区(海拔> 2 500 m)的育龄期女性68例作为高海拔组,居住在西宁市及海东市(海拔1 500 m~2 500 m)的育龄期女性68例作为中海拔组。检测两组的卵巢储备功能,随访生育力情况。结果:两组患者的体重指数、年龄、孕次、产次、受教育年限、婚姻年限等比较,差异无统计学意义(P> 0.05)。高海拔组卵巢体积小于中海拔组(P<0.05),高海拔组的窦卵泡个数(antral follicle count,AFC)、抗缪勒管激素(anti-müüllerian hormone,AMH)明显低于中海拔组(P<0.05)。高海拔组的血清促卵泡生成素(follicle-stimulating hormone,FSH)、黄体生成素(luteinizinghormone,LH)含量均高于中海拔组(P<0.05),血清雌二醇(estradiol,E2)含量低于中海拔组(P<0.05),两组血清催乳素(prolactin,PRL)含量比较,差异均无统计学意义(P> 0.05)。所有患者随访至2022年8月1日,平均随访时间为(16.20±1.22)个月。高海拔组妊娠率为47.06%,低于中海拔组的妊娠率(52.94%),但组间比较,差异无统计学意义(P> 0.05)。结论:高原低氧环境能影响女性卵巢储备功能与生育力,卵巢体积和卵泡数明显缩小,妊娠率降低,也可导致血清性激素分泌紊乱。Objective:This paper is aimed to investigate the effects of hypoxia on ovarian reserve function and fertility in women at different altitude of plateau.Methods:Sixty-eight cases of women of childbearing age who attended the outpatient clinic of our fertility center from May 2019 to January 2021 and live in Yushu and Guoluo regions(altitude>3000 m)were selected as the high-altitude group,and 68 cases of women of childbearing age who live in Xining and Haidong regions(altitude between 1900 m and 3000 m)were selected as the middle and high-altitude group during the same period.The ovarian reserve function of the two groups was tested,and fertility was followed up.Results:There were no significant differences in body mass index,age,parity,parity,years of education and years of marriage between the high altitude group and the middle and high altitude group(P>0.05).The antral follicle count(AFC),ovarian volume,and serum anti-millerian hormone(AMH)and antral follicle count(AFC)content in the high altitude group were significantly lower than those in the middle and high altitude group(P<0.05).The ovarian volume in the high altitade group is smaller than that in the middle altitude group(P<0.05).The contents of serum follicle-stimulating hormone(FSH)and Luteinizing hormone(LH)in the high altitude group were higher than those in the middle and high altitude group(P<0.05),and the serum Estradiol(Estradiol,P<0.05).E2 content was lower than that in medium-high altitude group(P<0.05),without significant difference in serum Prolactin(PRL)content between the two groups(P>0.05).All women were followed up until August 1,2022,with an average follow-up time of 16.20±1.22 months.The pregnancy rate of the high altitude group was 47.06%,which was lower than that of the middle and high altitude group(52.94%),but there was no significant difference between the two groups(P>0.05).Conclusion:The high altitude hypoxia environment,especially the high altitude,which can affect the ovarian reserve function and fertility of women,reduces the o
