摘　　要：目的:探讨血清转铁蛋白受体CD71水平对重症肺炎预后的预测价值。方法:选取2019年1月至2021年12月安庆市立医院收治的117例重症肺炎患者为研究对象,根据入科28 d内结局分为存活组和死亡组。收集患者一般资料和实验室检查指标。将预后结局(1=死亡,0=存活)作为因变量,血清CRP和CD71水平作为自变量,进行Logistic回归分析,构建两者联合预测模型。绘制ROC曲线,评估血清CD71、C反应蛋白(CRP)水平及两者联合对重症肺炎预后的预测效能。结果:28 d内死亡50例,病死率为42.7%。Logistic回归分析结果显示,血清CRP和CD71水平的OR(95%CI)分别为1.982(1.355~2.889)、3.615(2.654~4.575)。血清CD71、CRP水平及两者联合预测的AUC(95%CI)分别为0.831(0.752~0.911)、0.763(0.672~0.855)和0.848(0.773~0.923)。结论:血清CD71具有较好预测重症肺炎预后的效能。Aim:To investigate the predictive value of serum level of transferrin receptor CD71 for prognosis of severe pneumonia.Methods:A total of 117 patients with severe pneumonia treated in Anqing Municipal Hospital from January 2019 to December 2021 were selected,and were allocated into survival group and death group according to the prognosis within 28 days after admission.General data and laboratory test indicators were collected.Prognostic outcome(1=death,0=survival)was used as the dependent variable,and serum CRP and CD71 levels were used as independent variables to construct a Logistic regression model for combination prediction.ROC curve was drawn to evaluate the prediction efficacy of CD71,CRP and combination.Results:Within 28 days after admission,50 patients died,and the fatality rate was 42.7%.Logistic regression analysis showed that the OR(95%CI)of serum CRP and CD71 levels were 1.982(1.355-2.889)and 3.615(2.654-4.575),respectively.The AUC(95%CI)of serum CD71,CRP level and combination were 0.831(0.752-0.911),0.763(0.672-0.855)and 0.848(0.773-0.923),respectively.Conclusion:Serum CD71 has a good value in predicting the prognosis of severe pneumonia.
