摘 要:神经介入是脑血管疾病诊疗新兴的微创临床技术。选择合适的动脉建立通路是介入手术的基础和成功的关键。经股动脉入路和经桡动脉入路是神经介入诊疗的常规入路,但在临床应用中存在解剖相关局限性,难以适用于所有的患者。近期多项研究提示经尺动脉入路、经肱动脉入路以及经颈动脉入路等“非常规入路”可能是安全有效的选择。笔者现主要围绕不同动脉入路在神经介入诊疗中的应用进展进行综述,以期为临床介入医师提供参考。Neurointervention is an emerging and minimally invasive clinical technique for the diagnosis and treatment of cerebrovascular diseases.The selection of appropriate artery for the establishment of approach is the foundation and key to the success of interventional procedure.Transfemoral approach(TFA)and transradial approach(TRA)are the conventional approaches for neurointerventional procedures,but both have anatomical limitations and not applicable for all patients.With the development of interventional materials and techniques,several recent studies have suggested that“unconventional approaches”such as transulnar approach(TUA),transbrachial approach(TBA),and transcarotid approach(TCA)may be safe and effective option.This article mainly focuses on the application progress of different arterial approaches in neurointerventional diagnosis and treatment,in order to provide more references for neurointerventionalists.
