摘　　要：目的通过Meta分析方法评价活血化痰法中药治疗声带小结的临床疗效与安全性。方法计算机检索中国知网(CNKI)、维普数据库(VIP)、万方数据(WANFANG DATA)、中国生物医学文献数据库(CBM)、PubMed、Embase、Web of Science、Cochrane Library等中英文数据库中关于活血化痰法中药治疗声带小结的临床随机对照试验(RCTs)。检索时间自建库至2022年9月。根据Cochrane Collaboration标准对文献进行质量评价,采用RevMan5.4.1软件进行数据分析。结果纳入21项RCTs,共2170例患者,其中治疗组1181例,对照组989例。Meta分析结果显示,活血化痰法中药与不同干预方式的对照组对比,总有效率均显著高于对照组[OR=4.04,95%CI(3.16,5.15),P<0.00001],不同干预疗程活血化痰法中药组的疗效均高于对照组[OR=3.84,95%CI(2.97,4.97),P<0.00001];活血化痰法中药组声音嘶哑临床症状评分低于对照组[SMD=-1.15,95%CI(-1.79,-0.51),P=0.00004];活血化痰法中药组标准化噪声能量(NNE)嗓音声学参数低于对照组[MD=-4.84,95%CI(-7.96,-1.72),P=0.002];两组患者均未报道见严重不良反应。结论运用活血化痰法中药治疗声带小结患者疗效较好,可有效改善患者声音嘶哑临床症状及声带的闭合程度。Objective To evaluate the clinical efficacy and safety of activate-blood and disperse-phlegm method in the treatment of vocal nodules by Meta-analysis.Methods During the time of building the database to September 2022,databases such as CNKI、WANFANG DATA、VIP、CBM、PubMed、Embase、Web of Science、Cochrane Library were used to search the randomized controlled trials(RCT)for the treatment of vocal nodules with the activate-blood and disperse-phlegm method.The quality of the literature was evaluated according to the Cochrane Collaboration standard,and the data were analyzed by RevMan5.4.1 software.Results A total of 21 researches and 2,170 patients were included in this study.There were 1,181 patients in the experimental group and 989 patients in the control group.The results show:compared with the control group,the experimental group was much better in the improving total effective rate[OR=4.04,95%CI(3.16,5.15),P<0.00001].The curative effect of different intervention courses in the experimental group was higher than that in the control group[OR=3.84,95%CI(2.97,4.97),P<0.00001].The clinical symptom score of hoarseness in the experimental group was lower than that in the control group[SMD=-1.15,95%CI(-1.79,-0.51),P=0.00004],and the voice acoustic parameters of normalized noise energies(NNE)in the experimental group were lower than those in the control group[MD=-4.84,95%CI(-7.96,-1.72),P=0.002].No serious adverse reactions were reported in both groups.Conclusions The method of activate-blood and disperse-phlegm is effective in the treatment of vocal nodules,which can effectively improve the clinical symptoms of hoarseness and the degree of closure of vocal cords.
