糜蛋白酶溶液冲洗治疗先天性耳前瘘管感染并脓肿患者的效果观察

摘　　要：目的:探究糜蛋白酶溶液冲洗对先天性耳前瘘管(CPF)感染并脓肿患者治疗效果及复发的影响。方法:选取2018年3月—2020年4月于佳木斯市东风区人民医院耳鼻咽喉科接受治疗的CPF感染并脓肿患者60例作为研究对象,采用随机数字表法分为研究组(n=30)及对照组(n=30)。对照组对患处局部皮肤使用碘伏进行消毒,研究组采用糜蛋白酶溶液冲洗。比较两组临床疗效、症状改善情况、疼痛评分(VAS)、切口愈合换药时间及复发率、治疗满意度。结果:研究组治疗总有效率为93.33%,高于对照组的70.00%,差异有统计学意义(P<0.05);研究组感染控制时间、创面愈合时间、脓腔缩小时间短于对照组,差异有统计学意义(P<0.05);研究组治疗后VAS评分低于对照组,差异有统计学意义(P<0.05);两组5~7d及7~10 d换药切口愈合状况比较,差异无统计学意义(P>0.05),而研究组10~15 d换药切口愈合状况低于对照组,差异有统计学意义(P<0.05);研究组复发率为3.33%,低于对照组的30.00%,差异有统计学意义(P<0.05);研究组治疗总满意度为96.67%,高于对照组的76.67%,差异有统计学意义(P<0.05)。结论:糜蛋白酶溶液冲洗在CPF感染并脓肿患者治疗中可获得确切效果,能够有效缩短脓腔愈合时间及感染控制时间,缓解局部疼痛感,促进切口愈合,缩短切口愈合时间,且复发率低,安全性高,值得推广应用。Objective:To explore the effect of chymotrypsin solution washing on the treatment and recurrence of patients with congenital preauricular fistula(CPF)infection and abscess.Methods:A total of 60 patients with CPF infection and abscess who were treated in the otolaryngology department of Dongfeng District People’s Hospital of Jiamusi from March 2018 to April 2020 were selected as the study subjects.They were randomly divided into the study group(n=30)and the control group(n=30).The control group used Iodophor to disinfect the local skin of the affected area,and the study group used chymotrypsin solution to rinse.The clinical efficacy,symptom improvement,pain score(VAS),wound healing and dressing change time,recurrence rate and treatment satisfaction were compared between the two groups.Results:The total effective rate of the study group was 93.33%,higher than 70.00% of the control group(P<0.05).The infection control time,wound healing time and purulent cavity shrinking time in the study group were shorter than those in the control group(P<0.05).The VAS score of the study group was lower than that of the control group(P<0.05).There was no significant difference between the two groups in the healing status of dressing change incision at 5-7 days and 7-10 days(P>0.05),while the healing status of dressing change incision at 10-15 days in the study group was lower than that in the control group(P<0.05).The recurrence rate of the study group was 3.33%,lower than 30.00% of the control group(P<0.05).The total satisfaction of the study group was 96.67%,which was higher than 76.67% of the control group(P<0.05).Conclusion:Chymotrypsin solution irrigation can obtain a definite effect in the treatment of CPF infected patients with abscess,which can effectively shorten the healing time of pus cavity and infection control time,relieve local pain,promote wound healing,shorten wound healing time,and have low recurrence rate and high safety.It is worth popularizing and applying.
