摘 要:目的分析在临床抗生素合理应用中实施细菌耐药机制指导的效果及对多重耐药菌检出率的影响。方法将中国人民解放军联勤保障部队第九八八医院2020年3月至2022年8月使用抗生素治疗的患者(92例)作为研究对象,依据区间随机法将其分为对照组、研究组,各纳入46例,对照组治疗期间予以常规用药指导,研究组在常规用药指导前提下增加细菌耐药机制指导。比较两组不合理使用抗生素发生情况、抗生素使用知识掌握程度评分、多重耐药菌检出率、抗生素使用时间、抗生素使用费用、住院时间。结果研究组不合理使用抗生素发生率(4.35%)低于对照组(19.57%);研究组对抗生素使用知识掌握程度评分明显高于对照组;研究组多重耐药菌检出率(8.70%)低于对照组(26.09%);研究组抗生素使用时间、住院时间短于对照组,抗生素使用费用低于对照组,差异有统计学意义(P<0.05)。结论细菌耐药机制指导能有效促进抗生素合理应用,同时可降低多重耐药菌检出率,值得应用。Objective To analyze the clinical effects of the bacterial resistance mechanism guidance on the rational use of clinical antibiotics and the detection rates of multi-drug resistant bacteria.Methods A total of 92 patients of taking the antibiotic therapy from March 2020 to August 2022 at the 988th Hospital of the Joint Logistics Support Force of the People's Liberation Army of China were selected as research subjects.According to random interval method,patients were divided into a control group and a study group,each containing 46 cases.During the treatment period,the control group received guidance on routine medication,while the study group added guidance on bacterial resistance mechanisms under the premise of routine medication guidance.Compare the incidence of irrational use of antibiotics between two groups,the score of mastery of antibiotic use knowledge,the detection rate of multidrug-resistant bacteria,the duration of antibiotic use,the cost of antibiotic use,and the length of hospital stay Results The irrational usage ratios of antibiotic in the study group(4.35%)were significantly lower than control group(19.57%);the knowledge mastery scores of antibiotic usage in the study group were significantly higher than control group;the detection rates of multi-drug resistant bacteria in the study group(8.70%)were significantly lower than control group(26.09%);the antibiotic usage time and hospital stays in the study group were significantly shorter than control group;the antibiotic treatment costs were significantly lower than control group,and the difference was statistically significant(P<0.05).Conclusion The bacterial resistance mechanism guidance can promote the rational use of clinical antibiotics and reduce the detection rates of multi-drug resistant bacteria,making it worth applying.
