补气活血方联合防旋股骨近端髓内钉治疗股骨粗隆间骨折的效果

摘　　要：目的探究补气活血方联合防旋股骨近端髓内钉(PFNA)治疗股骨粗隆间骨折的效果。方法选取2019年1月至2020年1月于瑞金市中医院住院需手术的80例股骨粗隆间骨折患者作为研究对象,按照随机数字表法分为观察组与对照组,每组40例。对照组行PFNA治疗,观察组行补气活血方联合PFNA治疗,比较两组手术相关指标、术后并发症发生率及手术前后Harris评分。结果观察组住院时间、手术时间、首次下床活动时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为7.5%,低于对照组的27.5%,差异有统计学意义(P<0.05)。两组Harris评分组间、时间、交互比较差异有统计学意义(P<0.05);组内比较:两组术后各时间点Harris评分均高于前一时间点,差异有统计学意义(P<0.05);组间比较:术前,两组Harris评分比较差异无统计学意义;术后1、3、6个月,观察组Harris评分均高于对照组,差异有统计学意义(P<0.05)。结论补气活血方联合PFNA治疗股骨粗隆间骨折可降低患者手术不适,加快骨折早期愈合,促进患者关节功能恢复,降低术后并发症发生率。Objective To explore the effect of Buqi Huoxue Fang combined with anti rotation proximal femoral nail antirotation(PFNA)in the treatment of femoral intertrochanteric fractures.Methods 80 patients with femoral intertrochanteric fractures who were hospitalized and required surgery in Ruijin Traditional Chinese Medicine Hospital from January 2019 to January 2020 were selected as the research subjects,and they were divided into the observation group and the control group according to the random number table method,with 40 cases in each group.The control group received PFNA treatment,while the observation group received Buqi Huoxue Fang combined with PFNA surgery treatment,the surgical related indicators,incidence of postoperative complications,and Harris score before and after surgery were compared between the two groups.Results The hospitalization time,operation time,the first ambulation time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications was 7.5%in the observation group,which was lower than 27.5%in the control group,and the difference was statistically significant(P<0.05).There were significant differences in Harris score between groups of group,time potint and interaction(P<0.05).Comparison within group:the Harris scores of the two groups at each time point after surgery were higher than the previous time point,and the differences were statistically significant(P<0.05);comparison between groups:before surgery,there was no significant difference in Harris score between the two groups;at 1,3,and 6 months after surgery,the Harris score in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Buqi Huoxue Fang combined with PFNA in the treatment of femoral intertrochanteric fractures can reduce surgical discomfort of patients,accelerate early
