钻单孔冲洗联合T管挂壁引流术治疗慢性硬膜下血肿的临床研究

摘　　要：目的探讨钻单孔冲洗联合T管挂壁引流术治疗慢性硬膜下血肿(CSDH)的临床效果。方法选取2019年9月至2021年6月景德镇市第三人民医院收治的40例CSDH患者作为研究对象,按照随机数字表法分为研究组与对照组,每组20例。对照组采用保守治疗,研究组采用钻单孔冲洗联合T管挂壁引流术治疗,比较两组治疗成功率、住院时间、并发症发生率及血肿复发率。结果研究组治疗成功率为100.0%,高于对照组的70.0%,且住院时间短于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率为20.0%,血肿复发率为5.0%,均低于对照组的50.0%、30.0%,差异有统计学意义(P<0.05)。结论钻单孔冲洗联合T管挂壁引流术治疗CSDH效果较好,患者恢复速度快,手术难度较低,可有效降低并发症与复发率,值得临床广泛推广。Objective To investigate the clinical effect of drilling single hole irrigation combined with T-tube hanging wall drainage in the treatment of chronic subdural hematoma(CSDH).Methods 40 patients with CSDH in the Third People's Hospital of Jingdezhen from September 2019 to June 2021 were selected as the research subject,and they were divided into study group and control group according to the random number table method,with 20 cases in each group.The control group was treated with conservative treatment,and the study group was treated with drilling single hole irrigation combined with T-tube hanging wall drainage,the treatment success rate,length of stay,incidence of complications and hematoma recurrence rate were compared between the two groups.Results The treatment success rate in the study group was100.0%,which was higher than 70.0%of the control group,and the hospitalization time in the study group was shorter than that of the control group,the differences were statistically significant(P<0.05).The complication rate and hematoma recurrence rate in the study group were 20.0%and 5.0%,respectively,which were lower than 50.0%,30.0%in the control group,the differences were statistically significant(P<0.05).Conclusion Drilling single hole irrigation combined with T-tube wall drainage in the treatment of CSDH is effective,the recovery speed of patients is fast,and the difficulty of surgery is low,which can effectively reduce the complications and recurrence rate,and is worthy of clinical wide promotion.
