复方大黄汤熏洗联合外剥内扎术对混合痔患者治疗效果的影响

摘　　要：目的探究复方大黄汤熏洗联合外剥内扎术对混合痔患者治疗效果的影响。方法选取2019年1月至2020年4月本院收治的68例混合痔患者作为研究对象,采用随机数字表法分为两组,各34例。对照组单独行外剥内扎术治疗,观察组给予复方大黄汤熏洗联合外剥内扎术治疗,比较两组临床症状消失时间、术后疼痛程度、肛缘水肿程度、炎症因子水平、术后并发症发生情况。结果观察组水肿、便血、疼痛、肛门下坠消失时间均短于对照组,差异有统计学意义(P<0.05)。观察组术后疼痛和肛缘水肿程度均明显轻于对照组,差异有统计学意义(P<0.05)。术后4 h,观察组降钙素原(PCT)、C反应蛋白(CRP)和WBC水平均明显低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率(2.94%)低于对照组(23.53%),差异有统计学意义(P<0.05)。结论复方大黄汤熏洗联合外剥内扎术治疗混合痔效果显著,可有效改善临床症状,减轻炎症反应,减少术后并发症的发生,安全有效,值得临床推广应用。Objective To explore the effect of fumigation and washing of Compound Rhubarb decoction combined with external stripping and internal ligation on patients with mixed hemorrhoids.Methods 68 patients with mixed hemorrhoids treated in our hospital from January 2019 to April 2020 were selected as the study subjects,and they were divided into two groups according to random number table method,with 34 cases in each group.The control group was treated with external stripping and internal ligation alone,the observation group was treated with fumigation and washing of Compound Rhubarb decoction combined with external stripping and internal ligation,the disappearance time of clinical symptoms,the degree of postoperative pain,the degree of anal border edema,the level of inflammatory factors,and the occurrence of postoperative complications were compared between the two groups.Results The disappearance time of edema,hematochezia,pain and anal fall in the observation group were shorter than those in the control group,and the difference were statistically significant(P<0.05).The degree of postoperative pain and anal edema in the observation group were lighter than those in the control group,and the differences were statistically significant(P<0.05).At 4 h after operation,the levels of procalcitonin(PCT),C-reactive protein(CRP)and WBC in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group(2.94%)was significantly lower than that in the control group(23.53%),and the difference was statistically significant(P<0.05).Conclusion Fumigation and washing of Compound Rhubarb decoction combined with external stripping and internal ligation in the treatment of mixed hemorrhoids has significant effect,can effectively improve clinical symptoms,reduce inflammatory reaction and postoperative complications,and is safe and effective,which is worthy of clinical promotion and application.
